Requirements to apply:

• Licenses: MA RN License. (Unencumbered)   


• Certifications: CPR/BLS


• Education/Degrees: ADN, BSN Preferred


• Experience:  Min. of 1 years of RN experience, Strong OASIS experience is a MUST  


• Valid MA driver license and reliable transportation.                   

The Admissions RN conducts new patient assessments and admissions, to determine that the patient meets program eligibility requirements, as well as existing patient re-certifications. This position provides flexibility to visit patient homes and do follow up and scheduling from your home! 

Schedule: Monday – Friday

Times: 8 am to 5 pm 

***Payrate: $80,000 to 90,000 per year***

Responsibilities: 

• 
  

  The Admissions RN conducts home visits to new patients; obtaining a medical history, particularly as it relates to the present condition, from the client and/or family member(s). 

  
  


• 
  

  Productivity: Conduct min. of 12 - 20 admissions per week. Min case load is 35 visits per week., including recertification.

  
  


• 
  

  Conducts a physical examination of the client, including vital signs, physical assessment, mental status, appetite and type of diet, etc.

  
  


• 
  

  Evaluate the client, family member(s) and home environment/ situation to determine what health teaching will be required, assistance from the family that may be available, and how to best create a care plan for client.

  
  


• 
  

  Make recommendations to leadership for candidate admissions. 

  
  


• 
  

  In collaboration with other clinicians, the Admissions RN designs a plan of care for qualified patients.
