Children's Service Partnership (FSP) Program



Location: Los Angeles, 90011
Schedule: Monday – Friday 8am to 5pm
Pay: $25-$28/hour
Job Type: Temp, Temp to Hire (New Graduates Accepted)


Requirements: Bachelor’s or Master’s in Social Work, Psychology, Human Services, or similar fields preferred. Preferred 2 years’ experience but open to those who have recently graduated.


 


Job Summary


The Case Manager II will support continuous quality improvement of client care outcomes and the systems of delivering care. The position requires the ability to work with mentally dysfunctional clients and collaborate with community resources, ensuring that the needs of clients are met through case work principles and coordination.


 


Responsibilities



  • Understand the needs and behavioral characteristics of mentally dysfunctional clients.

  • Utilize community resources effectively to coordinate services.

  • Comply with the rules and regulations governing human services provider agencies.

  • Ensure confidentiality of client information and apply legal and ethical guidelines.

  • Utilize case work principles to deliver coordinated mental health services.

  • Conduct psychosocial assessments and evaluate community functioning of clients and families.

  • Monitor client adherence to coordination plans and manage the necessary flow of information.

  • Serve as a liaison between the company and community agencies, representing the company’s goals and policies.

  • Act as an advocate for clients, ensuring equitable support from human services systems.

  • Communicate professionally with clients and community representatives.

  • Identify and report service quality issues and contribute to finding solutions.

  • Maintain client confidentiality at all times.

  • Participate in fundraising and other special projects as needed.

  • Uphold the philosophy, goals, and objectives of the company.


Required Qualifications :


  • Education: Bachelor’s or Master’s Degree in Social Work, Psychology, Human Services, or similar fields preferred. Candidates with other degrees will be considered if they have case management experience.

  • Experience: Minimum of two years’ experience in human services as a Case Manager, Social Services Case Worker, Educational Psychologist, or a related field in a psychiatric environment, working with persistently and severely mentally ill clients is preferred. Open to recently graduated candidates.

  • Must be physically well, emotionally stable, and possess strong communication skills, both verbal and written.

  • Ability to work effectively with a multidisciplinary treatment team is essential.
