Requirements to Apply:
- Bilingual in English / Spanish is required to apply
- Certified Nursing Assistant License in CA - ACTIVE
- High School Diploma or GED
- Certifications: CPR or BLS, First Aid
- Experience: At least 1 year of experience as a home care aide or personal care attendant. At least 1 year of experience working with frail and elderly population

Pay Rate: $19.00 - $23.00 per hour depending on experience

Schedule:
-
- 40 hours weekly
- Monday to Friday - 8am - 5pm or 7am - 4pm (scheduled based on facility need)
Training Location: Training will take place in Reseda, CA lasting 2 weeks. Mileage between locations will be reimbursed during the training period.

Under the supervision of Center Coordinator, the CNA Care Attendant, provides personal care services to participants in the PACE Center and during transit to medical appointments according to departmental, para-professional and regulatory standards of care.

Responsibilities:

- Assists participants with bathing, grooming, toileting, feeding and other activities as needed while in the PACE Center.
- Accompany participants to medical appointments and trips to and from the PACE Center as needed.
- Routinely monitor vital signs such as weight, blood pressure, and pulse as ordered by the clinic.
- Maintain personal care supplies for participant and center such as incontinence supplies, linen, and personal grooming items.
- Work in conjunction with the Rehabilitation and Activities Departments to lead exercise and activities.
- Work with Dietitian to ensure participant is served diet-approved meals including monitoring food and fluid intake, and ensuring portable meals are sent home with the participant as approved by the IDT.
- Maintain a clean and sanity environment in the PACE Center at all times.
- Immediately report observed changes in participants behavior, health or mobility to the Center Coordinator and Nurse.
- Participate in the Interdisciplinary Care Team as required and communicate effectively with participants, family members, caregivers, staff and visitors in the PACE Center.
- Complete documentation in the medical record accurately and timely.
- Meeting and/or exceeding productivity targets and customer service targets and quality assessment targets.
- Demonstrate respect for and promotes participant rights including dignity, self-determination, access to care, confidentiality and independence.
