Requirements:



  • Minimum of one (1) year of documented experience working with a frail or elderly population; appropriate clinical placement experience may be substituted.

  • Associate’s degree in occupational therapy from an accredited program,

  • Active Certified Occupational Therapist Assistant license with the state of California.

  • Current CPR certification is required


Job Type: Full time- Temp or DH


 


 


Position Summary: 


Under the direction and supervision of the Supervising RN, provides occupational therapy services to the participants of the PACE center following the plan of care of the Occupational Therapist.


 


Responsibilities:



  • Assist the Occupational Therapist (OT) in treating participants with mobility and functional limitations.

  • Provide regular updates to the OT on participant progress.

  • Train participants on using adaptive and durable medical equipment.

  • Ensure participant and staff safety during therapy sessions.

  • Participate in relevant meetings (team, family, facility, hospital, etc.).

  • Follow the OT’s plan of care to provide treatment.

  • Educate staff, participants, and families on available therapy services.

  • Observe participants and report progress or decline to the OT.

  • Complete documentation, including daily notes and progress reports.

  • Adjust schedules as needed and travel to participant residences if required.




 


 


Required Qualifications :

Education & Training 



  • Minimum of one (1) year of documented experience working with a frail or elderly population; appropriate clinical placement experience may be substituted.

  • Associate’s degree in occupational therapy from an accredited program,

  • Active Certified Occupational Therapist Assistant license with the state of California.

  • Is medically cleared for communicable diseases and has all immunizations up to date before engaging in direct participant contact.

  • Current CPR certification is required.
