Purpose of Your Job Description


Under the supervision of the RN Clinic Supervisor, the Clinic Assistant is responsible for the development and implementation of home care services for program participants.


 


Delegation of Authority


As the Clinic Assistant, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.


 


Job Functions

Administrative Functions


1.     Provides Clinic front desk coverage: answers phones promptly and politely.


2.     Coordinates faxing of documents, triages phone calls appropriately, assists with rooming participants.


3.     Assists RN clinic supervisor as needed.


4.     Provides support to clinic staff


5.     Provides support to clinic coordinator as needed


6.     Orders all supplies for the Clinic and assists with stocking the clinic


7.     Orders all supplies for home care.


8.     Reviews pre-op checklist for completion


9.     Coordinates and implements home care services to assure that quality services are provided to meet participant’s needs.


10.  Coordinates all approved home care hours, DME and home care supplies with the participant, family, and appropriate company.


11.  Provides the utilization report for home care services and supplies


12.  Coordinates and authorizes all durable medical equipment and enters authorizations for all approved home care/DME in the electronic medical record (EMR). Delivers services in the EMR.


13.  Documents all interactions and ordered services in the electronic medical record.


14.  Works with supportive housing staff to ensure effective communication and coordination of care.


15.  Works closely with the Center Manager and Director of Quality and Compliance to receive, document and resolve reports of participant rights violations, grievances, appeals and other issues related to service and quality of care.


16.  Actively participates in Interdisciplinary Team meetings.


17.  Works closely with the Director of Quality and Compliance to oversee all activities related to quality assurance of Home Care.


18.  Works closely with the Center Manager and Director of Quality and Compliance to receive, document and resolve reports of participant rights violations, grievances, appeals and other issues related to service and quality of care.


 


19.  Participate in participant council meetings as assigned.


20.  Assist in the clinic as needed and assigned.

·       Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to the Center Manager. Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors and others.


·       Maintains competency according to departmental, organizational and professional standards. Attends and participates in workshops, seminars, etc., to keep abreast of current changes in the long-term care field, as well as to maintain a professional status.


·       Actively participates in the Quality Assessment and Performance Improvement (QAPI) process. Consistently meets or exceeds BCSC quality assessment and performance improvement targets.


·       Understands fiscal issues and is appropriately involved in meeting budget targets.


·       Continually seeks better ways for delivering services and communicating with participants while demonstrating proficiency in delivering age-specific care.


·       Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality, and independence.


·       Understands the importance of community involvement and participates as appropriate in activities that link BCSC to its communities.


·       Consistently meets or exceeds BCSC targets for productivity and customer service targets.


·       Effectively collaborates with staff peers and contractors to meet BCSC goals and further success.


·       Complies with all BCSC policies and procedures.


 


Participants’ Rights Functions


·       Maintains confidentiality of all participant information.


·       Ensures the participants’ right to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to wage complaints, are well established and maintained at all times.


·       Reviews participant complaints and grievances and makes written reports of action taken to the Administrator. Discusses with participant and family as appropriate.


 


SAFETY AND SANITATION Functions


·       Ensure that work areas are maintained in a clean and sanitary manner.


·       Participate in all fire safety and disaster preparedness drills in a safe and professional manner.


·       Ensure that all participant care rooms, treatment areas, and therapy rooms are maintained in a clean, safe and sanitary manner.


·       When working in the clinic, ensure nursing services work areas (including oxygen storage, nurses stations, and utility rooms) are maintained in a clean, orderly and sanitary manner.


·       Follow established policies and procedures, including appropriate dress codes, conduct, and use of proper body mechanics in lifting or moving participants or supplies, including the appropriate use of the Sara Lift, Hoyer lift, and gait belt.


·       Report all hazards/accidents immediately.


·       Report occupational exposures to blood, body fluids, infectious materials and hazardous chemicals to your supervisor.


·       Follow established ergonomic policies and procedures (e.g., a mechanical lifter is used when lifting or moving heavy objects).


 


Working Conditions


·       Works in office areas as well as throughout the facility and its premises. May drive to participant’s home or residence.


·       Moves intermittently during working hours.


·       Is subject to frequent interruptions.


·       Works beyond normal working hours, weekends and holidays and on other shifts/positions as necessary.


·       Is subject to call‑back during emergency conditions (e.g., severe weather, evacuation, post‑disaster, etc.).


·       Attends and participates in continuing educational programs.


·       Is subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as reactions from dust, disinfectants, tobacco smoke, and other air contaminants.


·       Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.


·       Communicates with nursing personnel, and other department personnel.


·       Is subject to hostile and emotionally upset participants, family members, personnel, visitors, etc.


·       Is involved with participants, family members, personnel, visitors, government agencies and personnel, etc., under all conditions and circumstances.


·       May be subject to the handling of and exposure to hazardous chemicals.


 


Education and Experience



  • Degree and diplomas: High school diploma.

  • Certificates, Licenses: Verification of completion of CPR (BLS) and first aid training. TB screening and successful completion of a health examination by a health care provider is required for employment. Valid California driver’s license.

  • Experience: At least one-year experience working with the frail or elderly. Experience in fast-paced clinic or physician’s office preferred.

  • Skills/Aptitudes: Must be able to work effectively as part of an Interdisciplinary Team. Must have excellent organization and customer service skills.


 


 Specific Requirements


·       Must be able to read, write, speak, and understand the English language.


·       Must possess the ability to make independent decisions when circumstances warrant such action.


·       Must possess the ability to deal tactfully with personnel, participants, visitors and the general public.


·       Must be knowledgeable of medical terminology, laws, and regulations as they pertain to long‑term care.


·       Must possess the ability to work harmoniously with other personnel.


·       Must possess the ability to minimize waste of supplies, misuse of equipment, etc.


·       Must be able to follow written and oral instructions.


·       Be knowledgeable in computers, data input and output.


·       Must be able to relate information concerning a participant’s condition.


·       Must not pose a direct threat to the health or safety of other individuals in the workplace.


 


Physical and Sensory Requirements


(With or Without the Aid of Mechanical Devices)


·       Must be able to move intermittently throughout the work day.


·       Must be able to speak and write the English language in an understandable manner.


·       Must be able to cope with the mental and emotional stress of the position.


·       Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.


·       Must function independently, have personal integrity, have flexibility, and the ability to work effectively with other personnel.


·       Must meet the general health requirements set forth by the policies of this facility which include a medical and physical examination.


·       Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.


·       May be necessary to assist in the evacuation of participants during emergency situations.
