MINIMUM QUALIFICATIONS TO APPLY:



  • Education: BS in Nursing preferred.

  • License/Certifications: Valid and unencumbered CA State RN license and valid California driver’s license. 

  • Experience: Minimum 3 years nursing experience with at least one year caring for the frail or elderly and 2 years in a NURSING LEADERSHIP role.  A Public Health Nursing background and/or experience with PACE or outpatient clinics is preferred. 

  • Skills/Aptitudes: Nursing knowledge and skills necessary to treat participants and manage complex clinical situations.  Ability to provide care for the frail elderly.  Ability to work independently and within an Interdisciplinary Team. Good organizational and supervisory skills. Must be a team player with a positive attitude who is able to oversee the team, be involved with committees, cover all positions in the clinic and provide direct patient care as needed.







Seeking a compassionate, mission-driven CLINIC MANAGER (RN) for PACE (Program of All-Inclusive Care for the Elderly). PACE is for ages 55+ and includes head-to-toe medical care, vibrant social centers, transportation services and  home care in Los Angeles County. Those living with family, alone, or in assisted living facilities may be eligible and most participants pay nothing! PACE Centers are a safe and enjoyable place for Seniors to spend the day keeping their health in check. Participants can join in for a delicious meal, fun activities, and make new friends while visiting for appointments with doctors, nurses, and therapists.




 


The CLINIC MANAGER (RN) role will be based at a brand new state-of-the-art facility in West Los Angeles (on West Pico Blvd). 


 







Position Summary:




Under the supervision of the Center Manager, the Clinic Manager (RN) will supervise, coordinate, evaluate and assist in the delivery of clinic nursing services in accordance with the company, state and federal standards for providing direct and indirect nursing care to PACE participants.  Works in close coordination with the Primary Care Physician to manage ongoing care and complex clinical situations and support the operation of the clinic.




The Clinic Manager (RN) will be responsible for 10-12 direct reports (RNs, LVNs, MAs, and Clinic Assistants).




Administrative Functions:



  • Assumes the administrative responsibility and accountability for the assigned duties of the Clinic Supervisor (RN). Coordinates nursing staff to assure that the  standard of nursing care is provided at all times.

  • Participates in conducting the initial and quarterly assessment of each participant’s health and social status; identifies overt problems/needs and sets priorities; treats problems requiring immediate referral and/or follow-up; and interprets and records latest diagnostic results.

  • In coordination with the  nursing staff and the Interdisciplinary Team, develops a Plan of Care based on nursing process that incorporates immediate and continuing care needs and includes the participant and family perspective.

  • Participates in providing care that conforms to accepted practice standards and age-related characteristics and needs of participants. 

  • Assists nursing staff in providing direct care that includes, but is not limited to wound care, IV therapy, blood sugar checks, IM injections, vital sign checks and/or follow-up on an episodic problem previously identified by the Primary Care Physician, such as congestive heart failure, chest pain, dehydration, edema, urinary tract infection or other health problems.

  • Actively participates in daily morning and Intake and Assessment meetings to update other Interdisciplinary Team members on changes in participants’ condition and movement between home, supportive housing, nursing home, hospital and other settings. Provides a briefing to Interdisciplinary Team members on participant health problems, daily schedules, planned tests, appointments, and medical specialty appointments scheduled.

  • Supervises and assists nursing staff as required in recording participant assessments, reassessments and care delivered in the electronic medical record.

  • Consults with and educates the participant, family members and/or caregiver regarding the disease process, self-care techniques, prevention strategies and medical interventions.

  • Provides the Primary Care Physician with a summary of health findings and any changes in a participant’s health condition.

  • Meet with department directors on a regularly scheduled basis, and conduct/participate in in-service classes and supervisory level training programs.

  • Attend and participate in workshops, seminars, etc., to keep abreast of current changes in the geriatric field, as well as to maintain a professional status.
