MINIMUM QUALIFICATIONS TO APPLY:



  • License: Valid and unencumbered RN License in State of CA

  • Certifications: BLS/CPR

  • Education: ASN or BSN

  • Language: Bilingual-Spanish/English required

  • Experience: At least 2 years of hands-on ACUTE CARE RN experience plus 1 year caring for the GERIATRIC population. Must have experience in a hospital, ER, Urgent Care or Outpatient Surgery Center at any time during career. Must have experience with IVs, bladder scans, and have the diagnostic abilities to assess and triage patients.


This is a hands-on, direct patient care role at a outpatient clinic dedicated to the underserved seniors in the community. Under the Supervision of the Clinic Nurse Supervisor, the Clinic RN collaborates with Primary Care Physician to manage care and complex clinical situations and support clinic operations in a state-of-the-art outpatient clinic setting.


 


FULL TIME, DIRECT HIRE (not a temp assignment or contract role)


 


SALARY: up to $110k




SCHEDULE: Monday- Friday - 8:30- 5:00 pm

Responsibilities: 



  • Conducts initial and quarterly assessment of each assigned participant’s health and social status; identifies overt problems/needs and sets priorities; treats problems requiring immediate referral and/or follow-up; and interprets and records latest diagnostic results.

  • In coordination with the Interdisciplinary Team, develops a Plan of Care based on nursing process that incorporates immediate and continuing care needs and includes the participant and family perspective.

  • Provides direct care that includes, but is not limited to wound care, IV therapy, blood sugar checks, IM injections, vital sign checks and/or follow-up on an episodic problem previously identified by the Primary Care Physician, such as Congestive Heart Failure, chest pain, dehydration, edema, urinary tract infection or other health problems.

  • Oversees the organization of the exam and treatment rooms and coordinates with other Interdisciplinary Team members to schedule participants in such a way as to assure day-to-day operational efficiency and participant well-being.

  • When assigned, participates in daily morning meetings to update other Interdisciplinary Team members on changes in participants’ condition and movement between home, supportive housing, nursing home, hospital and other residential settings.  Provides briefing to Interdisciplinary Team members on participant health problems, daily schedules, planned tests, appointments, and Medical Specialty appointments scheduled.

  • Records participant assessments, reassessments and care delivered in the medical record.

  • Consults with and educates the participant, family members and/or caregiver regarding the disease process, self-care techniques, prevention strategies and medical interventions.

  • Supports the Infection Control function in coordination with the Quality Assurance Coordinator.

  • Actively participants in the Quality Assessment and Performance Improvement (QAPI) process.

  • Coordinates with the Pharmacist and the Home Care Coordinator to support participant care.
