Full Time
Schedule: 8:30am-5:00pm, Monday - Friday

Job Duties:
Administrative Functions:
- Understands the External Pharmacy Review requirements as stated in the Quality Assessment and Performance Improvement (QAPI) Plan.
- Reviews participants’ drug regimen/profiles as needed
- Works with Medical Director to identify the most effective review processes, findings documentation, and prescriber education.
- Reports findings to the Quality Assessment and Performance (QAPI) Committee quarterly.
- Analyzes chart review findings to identify opportunities for physician education and outreach.
- Analyzes chart review findings to ensure appropriate medication management and utilization of medications.
- Applies chart review findings to clinical outcome focused projects.
- Works with contracted Pharmacy Benefit Management (PBM) firm to ensure efficient and accurate administration of the pharmacy benefit.
- Works with contracted pharmacy to assist with accurate PDE (prescription data event) reporting to CMS to meet all Medicare Part D requirements as needed.
- Identifies cost effective pricing for ingredient cost (brand & generic).Comprehensive utilization data from the PBM into our data warehouse.
- Reviews comprehensive utilization data from the PBM Management of our pharmacy network.
- Provides accurate and timely enrollment and disenrollment data to the pharmacy, contracted facilities, and staff. .
- Interfaces with contracted pharmacy on medication management issues.
- Ensures appropriate in-service for proper dispensing, storing and administering of participant medications.
- Reviews storage and system requirements and protocols for clinic and emergency medication supplies at least semi-annually or as needed.
- Provides oversight of refill process.
- Creates process efficiencies in clinical workflow around medication administration and processing.
- In coordination with the Health Plan Specialist, completes semi-annual and quarterly Medicare Part D reports as needed .
- Writes applicable policies and procedures as needed.
- Prepares for and attends Clinic staff meetings..
- Assists with drug usage guideline decisions at medical staff meetings by preparing and presenting utilization trends.
- Understands and optimizes policies and procedures that involve medications and biologics.
- Works with the Quality Assurance Coordinator to complete regulatory reports and filings as needed.
- Researches and responds to drug information inquiries from participants, families, and medical and administrative staff.
- Attends all committees as assigned.
- Lead P&T meetings quarterly.
- Complete an audit of Controlled House Stock Medication counts monthly.
- Completes home visits as needed for medication reconciliation and participant education.
Required Qualifications :
Requirements:
- Degree and Diplomas: Must possess a PharmD.
- Certificates, Licenses: Current California registered pharmacist license required. TB screening and successful completion of a health examination by a health care provider is required for employment. Must have a valid California driver’s license.
- Experience: 2 – 3 years working in managed care clinical pharmacy setting, clinic operations, or completion of pharmacy practice residency. At least 1-year experience working with the frail or elderly population(s).
- Skills/Aptitudes: Effective communication skills, both written and verbal, required. Computer literacy, especially with Microsoft Office products, is required. Accuracy, attention to detail, and consistent follow through are required. Willingness to work within the framework of a multidisciplinary team in the model of care. Sensitivity to the health beliefs and needs of a multi-ethnic aged population. Analytical skills preferred.
