In this role, you’ll manage both acute and chronic medical needs of patients while promoting preventive care through physical exams, screenings, and education. You’ll collaborate closely with experienced professionals and play a key role in ensuring continuity of care through thorough documentation and follow-ups. You will be integral to a team committed to quality, equity, and excellence in community health.

Key responsibilities include:

• Conducting physical exams and delivering preventive healthcare within established clinical guidelines


• Providing education, referrals, and coordination of care for patients and their families


• Ensuring timely and complete documentation in the electronic health record (EHR)


• Participating in peer review, quality improvement, and provider productivity efforts


• Collaborating with administrative leadership to support high-quality clinical care and access
