Job Overview

The healthcare provider ensures effective patient care plans by employing appropriate diagnostic procedures, therapies, consultations, and referrals, with a focus on achieving positive outcomes. They manage patient care continuity through scheduling, tracking return visits, completing treatment plans, and following up on chronic issues, abnormal tests, and missed appointments.
