Job Title: HCBA Waiver Social Worker (MSW)



Location: San Diego, CA 92114



Schedule: Monday – Friday, 8:00 AM – 5:00 PM (local travel required throughout San Diego County and to Imperial County as needed)



Pay: $37.05 - $42.03 - $47.80/hour (DOE)



Job Type: Full-Time, Temporary (approx. 4 months) with potential for Temp-to-Hire



Requirements: MSW required, Valid CA Driver’s License, Reliable Transportation




Position Summary:
Support underserved populations through in-home and community-based assessments and care coordination. This temporary role plays a vital part in the delivery of social work services under the HCBA waiver program, with the potential to convert to a permanent position.




Responsibilities:





    • Conduct in-home and community-based member assessments




    • Provide case management and care planning in collaboration with interdisciplinary teams




    • Maintain accurate documentation and timely reporting




    • Travel throughout San Diego County and Imperial County as needed




    • Participate in client evaluations, reassessments, and care plan updates




    • Ensure compliance with regulatory and program guidelines




    • Coordinate services and support for members and caregivers




    • Attend regular team meetings and case conferences







 


Required Qualifications :

Requirements/Qualifications:




  • Master’s degree in social work (MSW) from an accredited school




  • Minimum 2 years of experience in any of the following:




    • Managed care health plan




    • Utilization review, case management, or discharge planning




    • Transitional/acute care settings (e.g., critical care, LTAC, SNF, LTC)






  • Valid CA Driver’s License and access to an insured vehicle




  • Willingness and ability to travel within assigned regions




  • Bilingual English/Spanish highly preferred
