Full Time - DAYS ONLY
M-F 8:30am-5:00pm

Salary Range: $110,000 - $136,000

QUALIFICATIONS YOU MUST MEET TO APPLY:
- Bachelor’s Degree in Social Work, Nursing, or related Healthcare field
- 5 years healthcare experience, with at least 3 years supervisory experience, preferably in a geriatric care setting.
- Minimum of 1 year experience in working with the frail or elderly population.
- Demonstrated skills in leadership and decision-making.
- Demonstrated skills in effective oral and written communication.
- Must be hands-on! This is a highly visible and interactive role, NOT a desk job!

- Master's Degree
- Prior experience in working in a PACE Organization
- Fluent Bilingual-Spanish skills

JOB SUMMARY:
Under the supervision of the Director of Operations,The Health Center Director provides leadership to all aspects of the program, including social services, rehabilitation services, transportation services, clinic, dietary services, home care, recreation and activities, and medical records. This individual facilitates all aspects of the Interdisciplinary Team and Interdisciplinary Team Meetings, including assessment, care planning, care management, and utilization of services. The Health Center Director ensures that all staff are appropriately trained and perform duties and responsibilities within the PACE Organization. In coordination with the Director of Quality and Compliance, and Director of Marketing, the Health Center Director oversees all activities related to participant enrollment and disenrollment. There is a significant focus on participation in the Quality Improvement Program to ensure that services are provided in a manner supporting participant rights, demonstrating quality and safe care, and focusing on customer service and satisfaction. Responsibilities include demonstrating compliance with all Federal, State, and local regulations and licensing standards. It is important that the Health Center Director understands the significance of community involvement and participates in activities that link the organization to the community.
