Location: Greensboro, NC


Schedule: Full Time (split shift) or Part Time availability. Hours between 7am & 9pm.


Pay: $14-$15/hour - Depending on Experience


Job Type: Temporary Assignment


Job Summary
Under the direct supervision of a Registered Nurse (RN), the Home Care Aide provides home health care services to PACE Participants. Home Health aides will adhere to PACE Home Health guidelines per training protocols, policies, and procedures. Duties in the home include but are not limited to the monitoring and documentation of the Participant’s health condition, providing in-home support, and assisting with their activities of daily living (ADL’s).


Key Responsibilities:



  • Provide personal care assistance and support with daily living activities for assigned participants.

  • Assist with nursing care, restorative therapies, activities, and meal preparation.

  • Document participant health status and care activities accurately.

  • Collaborate with the Interdisciplinary Team (IDT) to ensure 24-hour care coordination.

  • Communicate effectively with participants, families, and healthcare staff.

  • Adhere to home care standards, policies, and regulatory requirements.


Work Environment:



  • Tasks may include assisting with mouth care, personal hygiene, and care for non-intact skin.

  • Potential exposure to bloodborne pathogens while assisting participants.


Required Skills and Abilities:



  • Ability to work in a team and communicate effectively.

  • Physical capability to lift and transport up to 50 pounds, climb stairs, and perform repetitive movements.

  • Ability to read, write, and communicate in English for effective documentation and care coordination.


Required Qualifications :

Specifications:



  • Education and Special Training: High school degree, or equivalent.

  • Certification: Currently listed as a Nursing Assistant (NA) I with the North Carolina Department of Health and Human Services (NC DHHS) with no substantiated findings of neglect, abuse, or misappropriation of resident’s property in a nursing home.


Experience:



  • At least 1 year of experience with aging, frail elderly.

  • 6 months clinical experience as a Nursing Technician, Nursing Assistant, Corpsman, or Emergency Medical Technician.


Licensure, Registry or Certification Required:



  • Certificate from an approved CNA training program.

  • Listed as a CNA I with the NC DHHS.

  • Valid NC driver’s license and vehicle.
