Job Summary:


 


The Home Care Coordinator is responsible for the support and implementation of home care services for participants. Provide support with the evaluation of a safe home environment through home visit assessments and coordination with home caregiver vendors and family caregivers, coordination of durable medical equipment, and nursing home care. Attends scheduled IDT and Coordination meetings. Responsible for acting as the liaison between the IDT and the home care providers, she or he should possess good leadership and communication skills. In addition, the Home Care Coordinator should be able to identify and understand participants’ medical and social needs in order to evaluate the home care needs of participants.


 


Key Responsibilities:


 


 1. Collecting information at the patient’s home and home safety needs of a frail elderly population of high-risk, and identifies and develops specific plans of care, the home care needs of a frail elderly population, and identifies and develops specific plans of care through home visits and coordination with home caregiver vendors, and family caretakers.


 2. Assess, identify, and collaborate with other members of the Interdisciplinary teams regarding all Durable Medical Equipment and Incontinence care needs of the participant. 3. Home assessments: home access and egress, ability to perform ADLs in the home environment, need for assistive devices, ability to summon immediate emergency assistance, relationship with co-habitants and neighbors.


 4. Collaborate with the Interdisciplinary Team (IDT) to evaluate and re-evaluate caregiver hours. 


5. Discuss and educate approved caregiver hours and assignment plan with home care vendor/ service provider, participant, family, and caregiver.


 6. Actively participates in Interdisciplinary Team (IDT) contributing to the participant plan of care. 


7. Perform any follow-up as instructed by the Provider & IDT.


 8. Attends staff, scheduled IDT and Care Coordination meetings. 


9. Document in accordance with Clinic departmental guidelines and within the established time frames. 


10. Act as a liaison between home care vendor and clinic related to participant’s needs. 11. Maintains patient confidence and by keeping information confidential. 


12. Complies with federal, state, and local legal and professional requirements by familiar with existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.


 13. Complies with policy and procedures of Clinic


. 14. Perform other duties as assigned.




Work Environment:


1. Exposure to medical clinic setting. 


2. Low risk contamination when adhering to Universal Precautions. 


3. Is required to drive car, with validated California Driver’s License and car insurance to attend meetings and other related functions.


 


Required Skills and Abilities:


 


1. Ability to work well with patients. Requires lots of patience, and a kind/caring attitude. 2. Must be able to manage/care of geriatrics. 


3. Must be able to work with frail, elderly patients with many chronic medical and psychosocial needs. 


4. Demonstrates respect for and promotes participant rights including dignity, self-determination, and access to care, confidentiality and independence.


 5. Must be able to work effectively in a team environment. 


6. Only act within the scope of his or her authority to practice. 


7. Experience and knowledge regarding the physical, mental and social needs of the patients highly desirable. 


8. Demonstrates respect for and promotes participant rights including dignity, self-determination, access to care, confidentiality and independence


Required Qualifications :
Specifications / Experience:


1. Graduation from an accredited certified nursing assistant program with a current CNA certified issued by the State of California is required. 

2. Minimum of 1 year experience practicing as a CNA or healthcare worker, and working with a frail or elderly population preferred. 

3. Current BLS certification also required. DL 

4. Bilingual: English/Spanish/Mandarin/Cantonese depending on location preferred.
