Home Care Manager (LVN)
Location: Eureka, California
Schedule: Monday through Friday 8a-5p onsite
Pay: 70k-87k annual, depending on experience
Job Type: Full time permanent with benefits
Requirements:
- At least two (2) years of experience as a RN/LVN required, preferably in home or community health care.
- One (1) year working with a frail or elderly population required.
- One (1) year of supervisory experience required.
Job Summary:
Under the supervision of the Director, the Home Care Manager is responsible for the development and implementation of home care services for program participants, including the coordination of durable medical equipment. The Home Care Manager directly supervises the in-home team.
1. Provides day-to-day oversight of home care services for senior participants.
2. Supervises the Home Care Assistant, Home Care Schedulers, and Home Care Supervisors in coordination of home care services.
3. Follows HSRC hiring practices including providing orientation, education, supervision and corrective action for the in-home care team.
4. Oversees and monitors the home care provided by in-home care contract agencies.
5. Works closely with the Center Manager and Quality Assurance Coordinator to receive, document and resolve reports of participant rights violations, grievances, appeals and other issues related to service and quality of care.
6. Using the Home Care Safety Assessment process, oversees and/or assesses the home care needs of frail elderly participants.
7. Ensures all assessments are completed and documented in a timely manner.
8. Conducts home safety visits as assigned to ensure completeness of the PACE enrollment process.
9. Works closely with home care agencies to orient, educate, supervise and evaluate, in-home caregivers.
10. Works closely with the Quality Assurance Coordinator to oversee all activities related to quality assurance of the home care program.
LICENSES OR CERTIFICATES:
1. Current Registered Nurse licensed by the California Board of Registered Nursing, or BVNPT Vocational Nurse Licensure required.
2. Current CPR and First Aid certification within six (6) months of hire.
3. Valid California Driver’s License, and personal auto liability insurance required. Must have a personal vehicle in good driving/operating condition and insured for State minimum liability requirements that can be used for the applicable job functions noted above.
PRE-SCREENING REQUIREMENTS:
1. Requires clearance of a DOJ and FBI criminal history background check
2. Pre-employment physical and PPD skin test.
3. Requires Motor Vehicle Report verification.
4. COVID-19 and Influenza (flu) vaccines preferred.
