HOME CARE NURSE (RN)

Brockton, MA

Must meet the following qualifications to apply:



  • Valid and unencumbered RN license from the State of Massachusetts.

  • Basic Life Support (BLS) certification.


  • One year of experience working with GERIATRIC patients.




  • Two (2) or more years of direct patient care experience in a home care environment.




  • Proof of COVID vaccination.






The Home Care Nurse (RN) provides all nursing functions according to policy and procedure and is responsible for medical case management and health services to home-based geriatric participants of the Elder Service Plan (ESP). 


 


The Home Care Nurse (RN) will travel within the neighborhoods surrounding the PACE Center in Brockton including Taunton, Attleboro, Foxborough, Mansfield, and Norton. Mileage Reimbursed! 


 


SCHEDULE: Full Time; Mon-Fri (occasional rotation for on-call)


HOURS: 8am-5pm 


SALARY: Up to $100K DOE


BONUS: $5k (sign-on)



 


RESPONSIBLILITIES:



  • Performs routine clinical nursing duties, including history taking, vital signs, administration of medications, dressing changes and other nursing procedures 

  • Responsible for Case Managing home based ESP participants through communication with staff members involved in participant care.

  • Completes nursing assessment and collaborates in the development of a comprehensive plan of care, as required by the Patient Care Team.

  • Triages patients’ regarding illness and medical therapy and advises the patient/caregiver of the most appropriate action in consultation with a provider whenever necessary.

  • Builds trust and fosters smooth relationships with participant's and their families.

  • Provides health teaching using appropriate information on health maintenance and disease management during each patient encounter according to ESP policies.

  • Must be able to travel to and from participants homes, long-term care facilities and the work site in a timely manner.  

  • Provides coaching and oversight to the Home Health Aides and Senior Home Health Aides.
