LOCATION: San Francisco, CA
JOB STATUS: Full time;
HYBRID ROLE: Work onsite 4 days/week and remotely 1 day/week
SALARY RANGE: Up to $150k (with at least 5 years of experience in a RN leadership role in Home Health/Home Care)

Are you a strong leader with experience managing caregivers and home health clinical operations? Our ideal candidate will have a solid understanding of home health regulations, experience in quality improvement, and the ability to build and lead teams, manage clinical operations, and ensure the effective coordination of services.
REQUIRED QUALIFICATIONS:
- Active, unrestricted licensure in California as a Registered Nurse (RN)
- A bachelor’s degree in nursing from an accredited program
- At least three years of experience in the past five years in a home health agency or home care operation at least one year of which was in a supervisory or administrative capacity.
- At least three years of management experience.
- At least one year of experience with frail older adults.
- Current BLS certification
- Interest and ability to work in a multilingual, multicultural environment.
PREFERRED QUALIFICATIONS:
- Master’s degree in Nursing (MSN) or a closely related field
- Familiarity with Title 22 Home Health regulations
- Prior experience working with a PACE program.
- Bilingual English/Cantonese.

ROLE OVERVIEW:
As Home Health Clinical Manager (RN), you will oversee and lead the day-to-day clinical operations and leadership of our home care and home health services.
KEY RESPONSIBILITIES:
- Oversee daily operations of the home care department.
- Provide leadership and direction to clinical staff, ensuring adherence to best practices and regulatory standards.
- Build and sustain a team of competent caregivers who provide quality and compassionate care.
- Collaborate with interdisciplinary teams to coordinate care and improve patient outcomes.
- Monitor performance metrics and implement quality improvement initiatives.
- Ensure compliance with federal, state, and local regulations governing home health services.
- Mentor and support staff to ensure a high level of patient care and satisfaction.
- Provide collaborative leadership for the PACE program with the management team.
