Requirements to Apply:
- Bilingual English / Spanish is required to apply
- LPN License in Massachusetts
- 1 year experience preferred
- Willing to consider New Grad LPNs with license in hand
- Must have reliable transportation, current registration, insurance coverage, and a valid state driver’s license
The LPN oversees the medical care of senior participants of the PACE program, in their homes or skilled nursing facilities with the goal to keep participants independent as long as possible.
Schedule: Monday- Friday - Full Time is 35 visit hours weekly
Weekends available if you choose to schedule yourself
Part time may be considered with commitment of 16+ visits weekly
Times: 8:00- 5:00 pm
Salary: $60,000 - $70,000 Annual + mileage reimbursement for travel!
Want to work more hours? Potential to exceed target visit hours and earn additional compensation!
Responsibilities:
- The LPN picks their patient load, target of 35 visit hours weekly for full time- visits typically last 20 minutes
- Participates in the planning and coordination of total client care while following nursing care plan.
- Observes, evaluates, and provides skilled nursing to the patient along with patient/family teaching.
- Documentation, data entry, meetings, and working well with others.
