QUALIFICATIONS TO APPLY:
- Graduation from an accredited Licensed Practical School of Nursing program
- Current MA LPN license required
- Current BLS (Basic Life Support for Healthcare Providers) Certification required
- 1 year of experience in geriatrics required
- 1 - 2 years of home care experience preferred
- Bilingual English and Spanish, Haitian Creole, Vietnamese, Portuguese or other language preferred
- Phlebotomy experience, particularly with a geriatric population desired

JOB SUMMARY:
The Licensed Practical Nurse (LPN) is a member of the Elder Service Plan Interdisciplinary Team (IDT) providing nursing care to patients in our ESP clinic and patients’ homes. The role of the LPN requires a high degree of flexibility as assignments may require supporting participant in their home or in the Adult Day Health Center. Under the direction of Registered Nurses (RNs), LPNs support nursing tasks in accordance with the patient's plan of care, all while building a trusting relationship with ESP Interdisciplinary Teams and patients.
Our LPN must be highly organized, adaptable, and have excellent relationship development skills. LPN's function under the direction and in partnership with Program RN's and must have demonstrated skill and competency in medication management, immunization administration, wound care, point of care testing, detailed clinical documentation, and clear communication.
JOB RESPONSIBILITIES:
- Performs clinical nursing duties including but not limited to set up, delivery and administration of medications; dressing changes; assistance with vaccination; obtaining vital signs; and other nursing procedures under the verbal or written order of a provider as prescribed by law, written clinical policies, and within the limits of licensure and training.
- Travels to patients’ homes or provides care in ESP clinic as assigned.
- Collaborates closely with ESP RN's around daily tasks.
- Communicates patient's status, new problems and/or indicators of significant status changes to the RN and other disciplines of the IDT.
- Interacts with patients and family members in a manner that adheres to the highest standards of care and professionalism.
- Supervises agency Home Care Aides’ adherence to the Home Care Plan.
- Attends IDT and/or family meetings as needed.
- Builds trust and fosters strong relationships with patients and their families.
- Completes required documentation same day or within expected timeframes.
- Complies with state regulations regarding continuing education requirements.
- Covers weekend shifts on an as needed/rotating basis.
- Must be able to travel to and from multiple patients’ homes and the work site in a timely manner.
