Job Description :

Provide direct patient care supporting Provider Care Teams in the Primary Care clinics. Assist the Provider in performance of procedures including wound care, immunizations, and medication administration, point of care testing, telephone prescription orders/refills, medication prior authorizations, maintaining patient EHR record and communication of test results. Provide safe and quality nursing care under the direction of the Provider or Community Health RN within LPN scope of practice.


 


Knowledge of: 


• The principles, practices, methods, and procedures of nursing as applied to community health nursing. • The uses and effects of medications and immunizations. • Issues of alcohol and drug abuse. • The "strength-based" model of behavior change. • Problem-solving techniques and negotiation skills in accordance with motivational interviewing and behavior change theory. • Utilization of community resources. • Medical record keeping practices. • Functions and resources of private and public agencies. • Electronic Health Record.


 


Skill or ability to:
