Licensed Vocational Nurse (LVN)

Job Description :
Summary: Responsible for wound care, recording vital signs, collecting specimens ordered, and preparing clients for physical exam rounds. Provides basic nursing care. Works under the supervision of a Registered Nurse.

 

Essential Duties and Responsibilities: 

1. Receives and reviews 24-hour report at the beginning of the shift. 

2. Administers and is accountable for controlled drugs per state regulations and company policies and procedures. Reports any discrepancies or errors to Registered Nurse immediately. Additionally, informs Director of Health Services immediately AFTER notifying Registered Nurse. 

3. Conducts walking rounds to determine persons supported status every two (2) hours and as needed, includes beginning and end of shift. 

4. Interacts with Direct Support Professionals (DSP), informing them of persons supported particular medical needs, treatments, precautions to be taken and direction and assistance as needed to provide optimum health care. 

5. Prepares medications for one scheduled administration time. Passes medication according to company policy and procedures which allows one (1) hour prior to and one (1) hour after scheduled medication. 

6. Maintains medication and treatment supplies as needed, including faxing new treatment and re-orders from pharmacy. Receives items and checks in and distributes on a timely basis according to Regulations. 

7. Communicates persons supported change of condition directly to physician when Registered Nurse is unavailable. Carries out phone orders, makes appropriate appointments, and informs Administrator, parents, guardians, Director, Director of Health Services, and Regional Center when appropriate. 

8. In the event of a serious incident, such as need to go to the hospital or Emergency Room, LVN must immediately notify parents, guardians, Administrator, Director, and Director of Health Services. 

9. Locks medication cabinet at all times with no exceptions. Only when the LVN is preparing medication, stocking the cabinet, etc. is the medication cabinet open and LVN must remain with the medications until the cabinet is locked. Maintains cleanliness and organization of medication closet throughout shift. 

10. Administers and records medication in a timely manner in Medication Administration Record (MAR) and progress notes as necessary. 

11. Documents in timely fashion treatments or nursing interventions entering into persons supported chart and/or Therapy as appropriate. 

12. Assists in initial evaluation of persons supported upon admission. 

13. Informs Registered Nurse/Nurse Administrator/Director of Health Services of all changes in persons supported condition which may require further nursing/medical assessment and charts same in persons supported chart and/or Therap. 

14. Assists in dining rooms during meals, as needed. Monitors special diets, feeding, assisting/training staff in proper feeding techniques. 

15. Completes incident reports when asked and gives to Administrator for final approval. 

16. Assists the clinical portion of ANT classes as needed. Will provide constructive feedback to Staff Development Coordinator/Instructor. 

17. Provides persons supported status updates in order for Registered Nurse to determine Day Program attendance. 

18. Escorts persons supported to dental and medical appointments as need arises. 

19. Assists doctors on monthly rounds to ensure safety and rights of persons supported as needed. 

20. Weighs all persons supported and documents appropriately. 

21. Communicates with other departments to ensure quality care. 

22. Completes all nursing documentation on a timely basis and by the end of shift

23. Understands and follows the facility fire/disaster procedures and participates in drills. Understands role to follow in case of any emergency. 

24. Uses proper body mechanics and observes all safety precautions to minimize injuries to self and persons supported. 

25. Practices, at all times, company no lift policy and procedures. 

26. Follows Health Care Plan as written by Registered Nurse. 

27. Follows assigned Charge Nurse’s directions in the event a Registered Nurse is not available. Reports to Charge Nurse any concerns. 

28. Understands and follows the Universal Precautions and Infection Control procedures. 

29. Understands and follows all company procedures and policies. 

30. Performs various other functions as assigned or as requested by Registered Nurse or Charge Nurse. 

31. Performs scheduled assignments involving Adult Day Program as part of nursing responsibilities. 

32. Informs Registered Nurse immediately of any significant occurrences and/or change in persons supported status. 

33. Therapy to be accessed only during work hours, no exceptions. 

34. Attends mandatory monthly staff meetings. 

35. Performs other duties as assigned to meet department and/or business needs.
Required Qualifications :
Education and/or Experience: Graduate of a Board of Vocation Nursing approved school with a current LVN license in California. Previous experience in the treatment of individuals with developmental disabilities, preferred.
