Licensed Vocational Nurse (LVN)

Location: Sacramento, CA

Schedule: Monday–Friday, 8:00 AM–4:30 PM

Pay: $35–$37 per hour (based on experience)

Requirements: 

Active CA LVN license, CPR/BLS certification, 1+ year of nursing experience (geriatrics or home health preferred), valid driver’s license and transportation

Job Type:

LVN will provide hands-on nursing care in a PACE program, supporting clinic operations and delivering patient-centered care to seniors.

Responsibilities:

  Provide direct nursing care under RN or physician supervision

  
  


• 
  

  Administer medications and monitor participant health

  
  


• 
  

  Conduct triage for acute needs following clinical protocols

  
  


• 
  

  Support care plan implementation and chronic disease management

  
  


• 
  

  Document treatments and changes in health in EHR

  
  


• 
  

  Assist with participant intakes, wound care, and wellness education

  
  


• 
  

  Collaborate with interdisciplinary team members

  
  


• 
  

  Ensure infection control and safety standards are upheld

  
  


• 
  

  Coordinate participant transitions between care settings

  
  


• 
  

  Support participant engagement and autonomy

  
  

• 
  

  Current California LVN license in good standing

  
  


• 
  

  CPR/BLS certification

  
  


• 
  

  Minimum 1 year of LVN experience (PACE, geriatrics, or home health preferred)

  
  


• 
  

  Experience with EHR systems

  
  


• 
  

  Strong communication and organizational skills

  
  


• 
  

  Valid driver's license and reliable transportation

  Bilingual (Spanish, Cantonese, Tagalog) a plus
