Salary Range: $109,700 to $130,400 Annually

About the Program:


The RCC Program is an interim housing program that provides bed space for eligible patients. To qualify, patients must be homeless, have an acute medical condition, and be medically and physically stable. This respite program offers medical care, case management, personal care, three meals per day, access to long-term housing resources and placement, and reunification services for clients who have lost contact with their family or support network. We also provides transportation for these patients to medical appointments, specialists, and other social service agencies within the county.




Requirements:



  1. Current RN license in the state of California. Bachelor’s degree in nursing (BSN) preferred.

  2. Minimum of 3–5 years of clinical nursing experience, with at least 3 years in a supervisory or management role.

  3. Maintain current CPR certification by American Heart Association on an annual basis (training may be provided).

  4. Possess a minimum of 4 years of experience in an ambulatory care setting.

  5. Skills:


    • Strong leadership and team management abilities.

    • Knowledge of harm reduction, trauma-informed care, and public health best practices.

    • Proficiency in medical documentation, reporting, and compliance management.

    • Experience working with underserved populations, including individuals experiencing homelessness and those with substance use disorders.

    • Excellent organizational, communication, and problem-solving skills.



Position Purpose:


The RN Nurse Manager plays a critical role in overseeing nursing operations at the Recuperative Care Center (LAG RCC). This position ensures the delivery of high-quality, patient-centered care in compliance with regulatory standards while supporting program leadership, nursing staff, and interdisciplinary teams in providing comprehensive medical services to individuals experiencing homelessness. Responsibilities include the supervision of Registered Nurses and Licensed Vocational Nurses. The Nurse Manager will receive guidance from the Program Director and clinical direction and updates from the Corporate Nurse Manager (CNM).




Principal Responsibilities:



  1. Provide clinical guidance and support to LAG-RCC staff, ensuring adherence to best practices in harm reduction and trauma-informed care.

  2. Oversee clinical operations, triage processes, dispensary management, and medication ordering.

  3. Conduct new hire orientations for nursing staff, including skills assessments.

  4. Develop, implement, and maintain comprehensive training programs for nursing and ancillary staff.

  5. Support the Program Director in overseeing nursing staff, addressing performance concerns, and ensuring compliance with agency policies.

  6. Partner with the Program Director to improve systematic functions, enhance service delivery, and address operational challenges.

  7. Assist in preparing reports on program operations, clinical performance, and programmatic goals.

  8. Monitor and report clinical compliance with safety guidelines and facility requirements.

  9. Participate in medical updates, staff meetings, and professional training sessions as required.

  10. Ensure adherence to JWCH policies and DHS regulations, supporting the agency’s mission to provide high-quality care to underserved populations.

  11. Conduct nursing assessments for new admissions and oversee patient screenings.

  12. Assist medical providers with procedures and ensure proper execution of treatment plans.

  13. Monitor medication management processes, supporting clients in self-administration and ensuring medication safety.

  14. Organize and maintain medication inventory, ensuring proper disposal of expired medications in coordination with LVNs.

  15. Ensure timely ordering and stocking of medical supplies.

  16. Provide patient education, facilitate specialist referrals, and coordinate hospital appointments as necessary.

  17. Ensure adherence to HIPAA regulations in handling protected health information (PHI).

  18. Support program leadership in meeting all outcome measures established by the Department of Health Services (DHS).

  19. Ensure compliance with federal, state, and local healthcare regulations, accreditation standards, and quality measures.

  20. Collaborate with the Operations and Quality Assurance teams to strengthen Patient-Centered Medical Home (PCMH) initiatives.

  21. Conduct internal and external audits, completing corrective action plans as necessary.

  22. Address, document, and resolve patient complaints following established procedures.

  23. Perform other duties as assigned.
