Job Overview
We are seeking a dedicated Family Nurse Practitioner or Physician Assistant to join a collaborative, patient-centered medical team. This full-time role blends preventive, chronic, and acute care in a primary care setting with a strong focus on continuity of care. The NP/PA will work closely with physicians and care teams to manage patient panels, perform diagnostic evaluations, provide education, and deliver comprehensive treatment plans. Responsibilities include SOAP note documentation, reviewing labs and referrals, participating in quality improvement, and engaging in team huddles. Limited rotational phone triage/on-call is required. Candidates must be compassionate, team-oriented, and eager to support our mission of increasing access to high-quality care.
