Job Overview
Under the supervision and direction of the Physician and Medical Director, the Nurse Practitioner provides primary care to our visiting Day Center participants by evaluating participant physical complaints and providing appropriate treatment. Some travel to SNFs or patient homes during the week, collaborating with a variety of caregivers.

Partner with a team that truly supports advancement, leadership development, and who welcomes your input for continuous program enhancements.
Requirements
- Master’s Degree in Nursing (MSN) and Board Certification
- Active and unrestricted Nurse Practitioner license required
- DEA State License
- Minimum two (2) year of post-graduation experience required, including some work with seniors. Seeking candidates who have a passion for working with senior populations
