Adult Geriatric Nurse Practitioner or Family Nurse Practitioner with Geriatric experience


Salary Range: $53.00 - $66.00/per hour (depending on experience) + Benefits


 


Requirements to apply:


        Graduate of an accredited Nurse Practitioner program with a current California RN and NP license, plus active DEA registration


        Minimum of five years of primary care experience, including at least three years serving elderly or medically complex populations


        Skilled in collaborative care; experience with interdisciplinary teams strongly preferred


        Valid California driver’s license, reliable insured vehicle, and current BLS/ACLS certification (or obtained within six months of hire)

Job Overview:


We are seeking a dedicated Nurse Practitioner to join our team in providing comprehensive, compassionate care to older adults. This role focuses on supporting seniors who wish to maintain their independence and continue living in their homes and communities. As part of a multidisciplinary team, you’ll deliver medical, social, nutritional, rehabilitative, and supportive services tailored to individuals’ needs. The goal is to enhance the quality of life, mitigate health crises, and promote overall wellness for our geriatric population. Provides professional medical services and ongoing care management for elderly patients within a community-based care model.
