Starting Salary Range: $127,700 - $151,100  



 


Requirements to apply:



  • Experience as an RN for 2 years with complex geriatrics populations in an outpatient clinic, prior to becoming an NP. 

  • Active, unrestricted NP license and CPR certification 

  • If NP, RN California State License 

  • Master’s degree and graduate of accredited NP/PA program 

  • Graduation from an Accredited School of Nursing 

  • Certificate as a Nurse Practitioner with Prescriptive Authority 

  • Current knowledge of chronic care/geriatric issues and best practices

  • Proof of a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law.  


Job Overview


This is an outstanding opportunity for a mission driven team player who is passionate about enabling seniors to meet their goals and live the independent lives they deserve. In this role, you will serve on an interdisciplinary team (IDT) and provide direct medical care to participants. You will work collaboratively with the IDT to create and drive forward a care plan that improves health outcomes for your panel of participants. In this role, you will meet participants where they are – whether that be in the center, clinic, home, virtually or an external site of care



  • Serves as a member of the IDT, completing sections of assessments and care planning, along with service determination request. 

  • Actively grows and manages a panel of participants.

  • Initiates, evaluates, reviews and changes orders for medications and diagnostic studies or treatments with approval. 

  • Assesses, initiates and coordinates care on a daily basis under supervision. 

  • Documents all participant interactions, assessments, next steps in the EMR within the specified timeframe set by the center. 

  • Makes referrals to specialists with approval and requests consults as needed. 

  • Performs history and physicals and participant teaching. 

  • Orders, interprets and follows up on lab tests and results. 

  • Performs selected procedures.  

  • Recognizes and escalates abnormalities as needed. 

  • Liaises with other team members for participant’s care, including behavioral health and pharmacy. 

  • Evaluates and treats participants where they are located, whether that be in the center, home, virtually, SNF etc. 

  • Facilitates advanced planning discussions with participants. 

  • Documents chronic conditions via HCC capture 

  • Provides on-call coverage as needed.
