Location: Stockton, California
Schedule: Monday through Friday 8a-5p
Pay: $60-63/hour
Requirements: Occupational Therapist with 1 year experience with frail & elderly, Bachelor degree
Job Type: 3 month temporary


 


Summary:


The Occupational Therapist (OT) is responsible for coordinating and providing restorative and rehabilitative Occupational Therapy services to participants at the Center and at home. Occupational Therapy works closely with PACE Physician, rehabilitation staff and other interdisciplinary team (IDT) members to maximize participant independence and safety as well as enhance performance of ADLs. The Occupational Therapist collaborates with the Physical Therapist to efficiently operate rehabilitation department and is a member of the management team and facilitates collaborative relationships across and with external stakeholders to promote the values, team culture and mission.


 


 


Responsibilities:



  • Conduct Occupational Therapy assessments to determine the Occupational Therapy care needs, preferences and goals of the participants and actively participate in IDT meetings to develop participant care plans.

  • Deliver and document Occupational Therapy interventions in accordance with physician orders and/or as agreed upon in the participants’ care plans including but not limited to the use of devices, techniques and activities to promote muscle re-education, perceptual-motor and cognitive function, balance, endurance and other factors affecting the level of independence in performing ADLs.

  • Supervises the Therapy Assistants including providing training, instruction and guidance needed to implement Functional Maintenance Programs (FMP).

  • Provide training and instructions to staff, participants and caregivers/families in areas of

  • Occupational Therapy in which they may work with participants.

  • Order, track and manage maintenance for equipment.

  • Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants’ condition or functional status.

  • Work with the other members of the IDT to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.).

Education/Training/Certifications:



  • Bachelor’s Degree in Occupational Therapy from Accredited Program AOTA and ACOTE standards.

  • Initial Registration from National Board for Certification in Occupational Therapy.

  • Licensed and/or eligible for Licensure as required in the State of California.

  • Valid State Driver’s License with a clean DMV record.

  • Copy of recent Vehicle Insurance required.


 


Skills and Experience:



  • Minimum of two years Occupational Therapy experience in a clinical setting with a frail or elderly population.

  • Occupational Therapy knowledge and skills necessary to treat frail, elderly participants with complex needs.

  • Highly motivated, inspirational leader who is able to innovatively engage, support and remove barriers for the team to ensure the team consistently meet established goals.

  • Two years’ supervisory experience with demonstrated ability to mentor and develop team members.

  • Ability to process accurate information in a timely manner.

  • Excellent leadership, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.

  • Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects.

  • Excellent clinical, organizational and communication skills in settings with seniors, their families and interdisciplinary team members.

  • Ability to work independently with minimal supervision.

  • Demonstrated ability to prioritize in a fast-paced environment.

  • Experience and competency working with people from diverse backgrounds and cultures.
