Location: Los Angeles, CA

Schedule: Full time Monday-Friday 8-5pm

Pay: $90/hr, depending on experience

Job Summary:

 
This is a role for a team player who is passionate about enabling elderly adults to live the independent lives they deserve. You will deliver occupational therapy to PACE participants in a variety of settings: care center, home and virtually. This includes delivering appropriate occupational therapy evaluation and treatment on acute, restorative and maintenance levels. In this role, you will serve as a member of the interdisciplinary team (IDT), working collaboratively to drive forward a participant’s care plan. You will partner closely with the Physical Therapist to efficiently operate the rehabilitation department at the PACE center.

 

 

 

Key Responsibilities: 



  • Serves as a member of the IDT, completing sections of assessments and care planning as well as reviewing service determination requests

  • Delivers occupational therapy to maintain or improve participant's baseline functional status and assesses participant's cognitive ability

  • Evaluates participants and recommends therapeutic exercises 

  • Develops plans of care depending on the needs of the participant and brings in participant and caregivers for feedback and sign off on goals 

  • Provides treatment according to protocols 

  • Educates participants and family members on procedures and methods to enhance mobility, safety, and functional independence of participants 

  • Conducts home safety evaluations as directed for prospective participants of the program  

  • Works with IDT to support care transitions between external settings and the home 

  • Assists in ordering, inventory, distribution and maintenance of durable medical equipment for participants 

  • Collaborates and oversees care within nursing home, skilled nursing facility and assisted living staff regarding the rehabilitation needs for participants 

  • Gives approval for care plan in the nursing home on behalf of the IDT 

  • Completes home assessments and treatments in home settings



Required Qualifications :

  • Passion and mission orientation for serving high-risk seniors and frail older adults  

  • Strong listener and communicator 

  • Team player 

  • Graduate of accredited Occupational Therapy program 

  • Occupational Therapy registration from National Board of Certification 

  • CPR and First Aid certification (within 60 days of employment) 

  • Valid California Occupational Therapy License, or proof of License eligibility 

  • Two (2) years of Occupational Therapy experience in a clinical setting with a frail or elderly population preferred 

  • Proof of a valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law 

  • Must be able to lift and carry or otherwise move up to 50 pounds 

  • Assist Participants with mobility challenges
