POSITION: Occupational Therapist

LOCATION: Fort Gratiot, MI

BRIEF SUMMARY OF PURPOSE OF POSITION:



Provide Occupational Therapy (OT) services to participants of Sunrise PACE at PACE location, and if needed, within their home environment. Assess, plan, organize, and participate in rehabilitative programs that help build or restore vocational, homemaking, and daily living skills, as well as general independence, to PACE participants. Act as part of PACE’s Interdisciplinary Team, within OT discipline to help assess and create care plans for participants based on their healthcare needs.

 

RESPONSIBILITES AND DUTIES:



  • Conduct physical assessment of participants, contributing to developing care/treatment plans, assessing home environments of participants, and developing physical rehabilitation programs.

  • Actively participates in team meetings by providing input in brainstorming sessions, sharing case studies, reviewing trends, and utilizing data feedback to improve processes and outcomes.

  • Act as member of Interdisciplinary Team (IDT) for OT. Make recommendations for participants, perform assessments and re-assessments as required. Participate in all required IDT meetings

  • Develop and maintain open lines of communication with staff.

  • Assessing home environments of participants and making determinations on what adjustments are needed.

  • Advising on adaptive equipment to help participants with daily activities.

  • Develop physical rehabilitation programs to help participants regain lost skills.

  • Develop and/or follow a treatment plan.

  • Educate caregivers and family members of participants on participant care

  • Evaluate results and progress of occupational therapy intervention on participants.

  • Maintain professional knowledge and technical progress in order to provide participants with the best treatment program available.

  • Comply with federal, state, and local certification requirements.

  • Evaluate participant condition regarding physical and basic mental health.

  • Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures that are in place.

  • Ability to drive and travel to residential appointments and activities to all counties of the PACE service area.

  • Must be able to read and interpret documents such as Medicaid policies and interpret supporting documentation.


 

QUALIFICATIONS AND SKILLS:



  • Graduate of an accredited program of occupational therapy

  • A bachelor's degree or above in Occupational Therapy

  • Licensure in the State of Michigan

  • Compliant with state healthcare regulatory requirements and certifications.

  • Up-to-date knowledge of treatment practices and physical therapy programs.

  • Excellent interpersonal communication skills.

  • Ability to initiate and maintain positive working relationships with staff, participants, contracted partners, and other organizations.

  • Must be accurate, conscientious and flexible with attention to detail and proficient with computers.

  • Ability to communicate with elderly and/or disabled participants on a one-to-one basis

  • Possess analytic and organizational abilities, interviewing and listening skills, assertiveness skills, and ability to work effectively as a member of an interdisciplinary team.

OSHA CATEGORY: It is unlikely that a person in this position in the regular course of business would encounter tasks or situations involving exposure to bodily fluids. Employee is required to attend Blood-borne Pathogens training on a regular basis as a precaution and to follow all guidelines of said training if an event should ever present itself.
