PACE CLINIC RN

Location: San Jose, CA

Schedule: Full-Time

Pay: $68.80 - $79.35 per hour (DOE)

Requirements: Fluent in Vietnamese or Chinese Required

Job Type: Full-Time, open to temporary, temp-to-hire, or permanent positions.

Position Summary:

The PACE Clinic RN works under the supervision of the PACE Center Manager, providing direct and indirect nursing care to PACE Participants. This role involves collaboration with physicians, clinic staff, and the Interdisciplinary Team (IDT) to manage patient care in a primary care setting within the PACE Center. The RN ensures quality care, patient education, and smooth care transitions.

Key Responsibilities:

1. Conduct patient assessments to determine care needs and goals.


2. Participate in IDT meetings for care planning.


3. Administer medications, treatments, and tests.


4. Monitor and update care plans based on patient conditions.


5. Coordinate patient transitions between care settings.


6. Educate patients, families, and caregivers on health management.


7. Assist with new enrollments and end-of-life care.


8. Respond to medical emergencies within RN scope.


9. Accurately document care in Epic EMR.


10. Supervise and support LVNs and MAs for efficient care.

• BLS & First Aid Certification


• Graduate of an Accredited Nursing Program


• Fluent in English & Vietnamese or Chinese
