Schedule: Monday – Friday, 8:00 AM – 5:00 PM

Pay: 22.39 - $28.09 (DOE)

Requirements: CNA & HHA certification, CPR/First Aid (AHA)
Job Type: Full-Time, Direct Hire

Position Summary:
Coordinates non-skilled home care services for elderly PACE participants, including safety checks and service delivery, in collaboration with the care team (IDT).

Responsibilities:

• 
  

  Coordinate delivery of meals, DME, and caregiver services

  
  


• 
  

  Conduct home safety evaluations (pre-enrollment, annual, etc.)

  
  


• 
  

  Maintain accurate documentation in EMR

  
  


• 
  

  Participate in care plan development and team meetings

  
  


• 
  

  Communicate with families, caregivers, and external vendors

  
  


• 
  

  Perform monthly visits for homebound and RCFE participants

  
  


• 
  

  Support IDT with updates and care coordination

  
  


• 
  

  Deliver supplies/equipment as needed

  
  


• 
  

  Attend training and follow care policies/procedures

  
  


• 
  

  Monitor service delivery quality and recommend changes

  
  

• 
  

  CNA and HHA certification (CA Dept. of Public Health)

  
  


• 
  

  CPR and First Aid (American Heart Association only)

  
  


• 
  

  1+ year experience with frail/elderly populations

  
  


• 
  

  Valid CA Driver’s License

  
  


• 
  

  Basic computer skills (Microsoft, Outlook, EMR)
