Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Pay: 22.39 - $28.09 (DOE)
Requirements: CNA & HHA certification, CPR/First Aid (AHA), 1+ year with elderly, Valid CA Driver’s License
Job Type: Full-Time, Direct Hire

Position Summary:
Provides non-skilled home care coordination for elderly participants in the PACE program. Supports care planning, conducts safety evaluations, and ensures delivery of services like meals, caregivers, and medical supplies in collaboration with the IDT.

Responsibilities:

• 
  

  Coordinate delivery of meals, DME, and caregiver services

  
  


• 
  

  Conduct home safety evaluations (pre-enrollment, annual, etc.)

  
  


• 
  

  Maintain accurate documentation in EMR

  
  


• 
  

  Participate in care plan development and team meetings

  
  


• 
  

  Communicate with families, caregivers, and external vendors

  
  


• 
  

  Perform monthly visits for homebound and RCFE participants

  
  


• 
  

  Support IDT with updates and care coordination

  
  


• 
  

  Deliver supplies/equipment as needed

  
  


• 
  

  Attend training and follow care policies/procedures

  
  


• 
  

  Monitor service delivery quality and recommend changes

  
  

• 
  

  CNA and HHA certification (CA Dept. of Public Health)

  
  


• 
  

  CPR and First Aid (American Heart Association only)

  
  


• 
  

  1+ year experience with frail/elderly populations

  
  


• 
  

  Valid CA Driver’s License

  
  


• 
  

  Basic computer skills (Microsoft, Outlook, EMR)
