Minimum requirements to be considered:



  • High School Diploma, GED or six (6) months experience as a personal care aide

  • One year experience assisting with the frail or elderly population


  • License: Current CNA



  • Bilingual (English/Spanish)

  • Valid California Driver’s License

  • Basic computer skills, Microsoft, Outlook, Excel


Must possess the skills necessary to provide care for geriatric patients and must be able to function well in a team environment. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by the PACE program and approved by CMS before working independently.


 


Schedule: Monday to Friday, 8 AM to 5 PM


Pay Rate: $19.46 to $26.19


 


Summary: Under the supervision of the PACE Home Health RN, the Home Health Aide provides support to the Home Health team in the coordination and delivery of home care services to PACE participants. Home Services include: Durable Medical Equipment (DME), delivery of meals, caregiver services, lifeline, ramps, medical supplies, oxygen tanks, etc. Position will assist with home delivery of meals and medical supplies. Position may also assist in other areas such as primary care and therapy.


 


Essential Functions of the Job:



  • May provide or coordinate for direct personal care to participants as directed or assigned by the Home Health Nurse.

  • The Home Health Aide duties if providing direct care at the home may include showering, toileting, laundry, feeding assistance, and companionship services to and from medical appointments.

  • Assists other staff in carrying out program assignments.

  • Provides support to PACE nurses in the coordination and delivery of meals, DME, caregiver services, etc.

  • Assists in maintaining patient areas in a clean and orderly manner.

  • Assists with home delivery of meals and medical supplies.

  • Follows infectious disease precautions to prevent the spread of organism.

  • Attends center staff meetings and in-service educational training programs.

  • Conducts home visits
