PACE Occupational Therapist
Location: Vista, CA
Schedule: Monday- Friday, 8:00 AM - 5:00 PM
Pay: $43.00 -$56.00 (DOE)
Requirements: Licensed Occupational Therapist in CA, 1+ year OT experience with geriatric population, CA driver’s license & transportation, bilingual English/Spanish preferred
Job Type: Full-Time, Temporary (LOA Coverage - July 9th -Nov 19, 2025)

Position Summary:
Temp, full-time role providing coverage while the current OT is on leave. The OT will work closely with a multidisciplinary care team to assess and support elderly participants in maintaining independence and improving function.

Key Responsibilities:

  Assess participants' functional abilities and develop individualized care plans

  
  


• 
  

  Deliver therapeutic activities and interventions to support mobility and independence

  
  


• 
  

  Educate participants and caregivers on use of adaptive equipment and safety techniques

  
  


• 
  

  Monitor progress and adjust care plans as needed

  
  


• 
  

  Conduct home and community site visits when required

  
  


• 
  

  Document assessments, treatments, and progress notes accurately

  
  


• 
  

  Collaborate with interdisciplinary team to ensure coordinated care

  
  


• 
  

  Participate in staff training and development sessions

  
  


• 
  

  Ensure compliance with all regulatory standards (PACE, CMS, DHCS)

  
  


• 
  

  Maintain appropriate therapy supplies and equipment

  
  

• 
  

  Graduate of accredited OT program

  
  


• 
  

  Licensed Occupational Therapist in the state of CA

  
  


• 
  

  Minimum 1 year of OT experience working with geriatric populations

  
  


• 
  

  Bilingual English/Spanish highly preferred

  
  


• 
  

  Valid CA driver’s license and reliable transportation

  
  


• 
  

  Proof of current auto insurance (if applicable)

  
  


• 
  

  Strong interpersonal and communication skills

  
  


• 
  

  Knowledge of adaptive equipment and rehabilitation practices
