PACE Social Work Supervisor

Location: El Cajon, CA

Schedule: Full-Time, Onsite

Pay: $39.93/hr – $55.28/hr (depending on experience)

Requirements: Master’s degree in social work (MSW)

Position Summary:
Supervises and leads the Social Services team at a San Diego PACE site, ensuring quality delivery of social work services, regulatory compliance, strong interdisciplinary collaboration, and effective case management for elderly participants. Oversees daily operations, develops staff, and occasionally carries a participant caseload.

Responsibilities:

  Supervise, coach, and manage social workers and case managers.

  
  


• 
  

  Ensure timely and accurate participant assessments and care plans.

  
  


• 
  

  Maintain quality assurance and documentation compliance.

  
  


• 
  

  Participate in and contribute to interdisciplinary team (IDT) meetings.

  
  


• 
  

  Liaison with families, caregivers, and community resources.

  
  


• 
  

  Provide crisis intervention support as needed.

  
  


• 
  

  Monitor department outcomes and report productivity weekly.

  
  


• 
  

  Conduct home visits for assessments and participant support.

  
  


• 
  

  Act as site program administrator when required.

  
  


• 
  

  Train staff on clinical and documentation best practices.

  
  

• 
  

  MSW from an accredited institution (required)

  
  


• 
  

  1+ year social work experience in healthcare (required)

  
  


• 
  

  Valid CA driver’s license, reliable vehicle, and insurance (required)

  
  


• 
  

  First Aid/CPR Certification (required)

  
  


• 
  

  EPIC EMR experience (preferred)

  
  


• 
  

  Leadership or supervisory experience (preferred)

  
  


• 
  

  Strong communication, organization, and independent work skills

  Comfort traveling across San Diego County for participant visits
