Requirements to Apply:
- Bilingual: English & Cantonese Required to apply (May accept Mandarin, may consider Vietnamese)
- Education/Degrees: Bachelor, Master or Doctoral degree approved by the California Board of Physical Therapy
- Licenses/Certifications: Licensed to practice as a Physical Therapist in California; Current BLS and First Aid required
- Years of experience: A minimum of 6 months of current or past PT experience and must meet the educational requirements set by the California Board of Physical Therapy; must be a fast learner and strong teamwork
- Preferred experience:
- At least one year of clinic experience to qualify for clinic setting
- At least one year of experience working with the older adult population.
- Two to five years professional experience in Physical Therapy in a health care setting (not including internship).
Schedule: Monday – Friday; 40 hours a week
Times: 8:30AM to 5:30PM
Location: Chinatown San Francisco, CA
Pay Rate: $112,247.20 - $128,752.00 (Depending on experience)
Sign-On Bonus: Up to $20,000
The Physical Therapist is independent in the application of physical therapy knowledge and skills and is responsible for evaluating and assessing the needs of patients / participants and formulating treatment plans. The Physical Therapist provides therapy services defined in treatment plans and works cooperatively with physicians in the PACE Program or FQHC Clinic setting.
Responsibilities:
- Conducts initial and periodic occupational therapy assessments to determine PACE Participants’ care needs, preferences, and goals.
- Remains alert to pertinent input from other team members, Participants, and caregivers and updates the IDT promptly regarding any changes in Participants’ condition or medical status.
- Evaluates participants by applying diagnostic and prognostic procedures; evaluates participant’s rehabilitation potential.
- Participates in the development and revision of the participant’s plan of care as an active member of the interdisciplinary team.
- Develops and implements a rehabilitation program to promote, restore or maintain participant’s optimum level of functioning, ensuring integration into interdisciplinary plan of care.
- Conducts an ongoing evaluation of the participant’s rehabilitation program to assess the progress and effectiveness of treatment.
- Reports participant’s treatment response to the PACE interdisciplinary team; changes treatment plan and goals, accordingly.
- Provides direct physical therapy for participants.
- Instructs participant and their decision-makers regarding in-home exercise program, gait training, transfers, ADL care, home modification, and on the care and use of equipment.
- Consults with participant and their decision-makers regarding individual expectations, as well as, long- and short-term goals
