Full Time Position

- Bachelor' degree in Physical Therapy if obtained prior to the year 2000; master's degree in physical therapy if obtained after the year 2000.
- Current licensed physical therapist to practice in CA.
- Minimum of 1 year working with Frail or Elderly population preferred
- Must have reliable transportation, a current driver’s license, proof of auto insurance.
- Must be willing to visit Participants of the PACE Program in their homes if requested to do so.
Schedule: Monday – Friday
Shift Time: 8:00AM to 4:30PM
Location: Fresno, CA
Pay Rate: $90,000 - $110,000 annually (Depending on experience)
Mileage Reimbursement also included for any in home services provided!
The physical therapist for PACE evaluates and assesses participants to determine mobility level and other physical conditions and functional abilities. Provides and recommends therapeutic exercise, gait training, ambulation, etc. Plans and develops individual and group exercise programs for maintenance therapy for participants. Responsible for providing a comprehensive physical therapy program for participants of PACE, including development and oversight of functional maintenance programs.
Responsibilities:
- Assessment: Collects patient data and completes required forms with appropriate responses according to department and industry standards; identifies participant's overt problems/needs and sets priorities. Identifies problems requiring further referral and/or follow up; observes and records latest diagnostic results; performs advanced physical therapy observations using critical thinking skills.
- Care Planning: Develop a plan of car based on physical therapy process incorporating plans of other disciplines and continuing care needs, including that participant and family in development and refinement of plan. Makes referrals to interdisciplinary support services. Communicate participant changes to team members.
- Care Management and Implementation: Ensure that therapy being provided conforms to accepted industry practice standards and organizational/department standards. Explain physical therapy treatment in appropriate way. Document all physical therapy treatment accurately and within departmental time requirements into patient's charts. Understand and demonstrate respect for participant's rights and utilize established mechanisms for management of ethical issues in participant care. Plan and develop maintenance exercise and therapy programs for participants.
- Participant/family education: Provide education to participant's family and team in the coordination of 24-hr care delivery. Identify emergency situations and independently initiate advanced therapy.
- Evaluation: Record patient care delivered as planned and any variation, with appropriate rationale. Make and record observations, as well as implement rehabilitation measures related to impending or associated problems.
- Leadership: Provide supervision to program aides and other staff who are implementing a physical therapy treatment and/or maintenance program; integrate cost effective measures into rehabilitation practice; recognize unit problems and take responsibility for documentation; demonstrate active participation in QI process; complete written reports; fulfill call for department as assigned and adjusting staffing for volume and acuity as necessary.
- Interdisciplinary Team (IDT): Participate as an active and cooperative member of the IDT Team. Attend staff meetings and take part in participant care planning.
- Additional duties and projects as assigned.
