JOB TITLE: Physical Therapist (PT)


Location: Victorville, CA


Schedule: M-F 8a-5p


Pay: $46/hr+DOE


 



  • Requirements: At least one year of documented experience working with a frail or elderly population as a physical therapist (clinical placement experience may be substituted).

  • A Bachelor's degree in Physical Therapy from an accredited program and a current California license.

  • Current CPR certification and are medically cleared for communicable diseases with up-to-date immunizations.

Position Summary: Come join Cross Country Workforce Solutions Group (CCWSG) at one of our partnering PACE programs where you will be a vital part of our interdisciplinary team, delivering top-notch physical therapy services. Your role will involve assessing, planning, and implementing rehabilitative programs to address mobility issues, improve functional abilities, and elevate the overall well-being of our participants, empowering them to live fulfilling lives in their communities.


 


Responsibilities:


 



  • Conducting initial, semi-annual, unscheduled, and annual assessments.

  • Participating in daily updates and reporting status changes.

  • Developing and implementing QAPI (Quality Assessment and Performance Improvement) activities.

  • Providing regular progress updates to the team.

  • Assessing needs for DME (Durable Medical Equipment), ordering, and facilitating authorizations.

  • Attending meetings (morning, team, family, nursing facility, ALF, hospital) to ensure comprehensive care.

  • Delivering treatments and ensuring safety measures are followed.

  • Creating special programs and educating staff, participants, and families on available services.

  • Supervising and training PTAs (Physical Therapist Assistants) and restorative aides.

  • Reviewing and revising restorative exercises as needed.

  • Conducting orientation and training on body mechanics and lift equipment.

  • Completing documentation and maintaining confidentiality.

  • Adjusting schedules to meet participant needs, including providing home therapy services as needed.


 


Required Qualifications :


  • At least one year of documented experience working with a frail or elderly population as a physical therapist (clinical placement experience may be substituted).

  • A Bachelor's degree in Physical Therapy from an accredited program and a current California license.

  • Current CPR certification and are medically cleared for communicable diseases with up-to-date immunizations.

  • The ability to cope with mental and emotional stress, work independently, and collaborate effectively with team members.

  • Excellent communication skills, organization, dependability, flexibility, and resourcefulness.

  • A valid California driver's license with a good driving record (for home visits).
