JOB TITLE: Physical Therapist (PT)
Location: Victorville, CA
Schedule: M-F 8a-5p
Pay: $46/hr+ DOE
Requirements:
- At least one year of documented experience working with a frail or elderly population as a physical therapist (clinical placement experience may be substituted).
- A Bachelor's degree in Physical Therapy from an accredited program and a current California license.
- Current CPR certification and are medically cleared for communicable diseases with up-to-date immunizations.

Your role will involve assessing, planning, and implementing rehabilitative programs to address mobility issues, improve functional abilities, and elevate the overall well-being of our participants, empowering them to live fulfilling lives in their communities.
- Conduct initial, annual, and unscheduled participant assessments.
- Provide regular progress updates and report status changes.
- Develop and implement QAPI (Quality Assessment and Performance Improvement) activities.
- Assess needs for DME, order equipment, and facilitate authorizations.
- Attend meetings to ensure comprehensive participant care.
- Deliver treatments while ensuring safety measures are followed.
- Educate staff, participants, and families on available therapy services.
- Supervise and train PTAs and restorative aides.
- Complete documentation and maintain confidentiality.
- Adjust schedules as needed, including providing home therapy services.
Required Qualifications :
- At least one year of documented experience working with a frail or elderly population as a physical therapist (clinical placement experience may be substituted).
- A Bachelor's degree in Physical Therapy from an accredited program and a current California license.
- Current CPR certification and are medically cleared for communicable diseases with up-to-date immunizations.
- The ability to cope with mental and emotional stress, work independently, and collaborate effectively with team members.
- Excellent communication skills, organization, dependability, flexibility, and resourcefulness.
- A valid California driver's license with a good driving record (for home visits).
