Are you passionate about delivering innovative solutions for older adults to live a meaningful, independent life at home with grace and dignity? Look no further! Our Program of All-Inclusive Care for the Elderly (PACE) is seeking a dedicated Physical Therapist to join our team.

We prefer a Physical Therapist that is licensed in both Kentucky and Indiana as you will be covering two of our facilities as needed. However, we are open to hiring a PT for Indiana and a PT for Kentucky!

Position Summary:
As a Physical Therapist with our PACE program, you'll play a vital role in evaluating participants, administering physical therapy treatments, and recording progress. You'll work both in our PACE Center and in participants' homes, coordinating 24-hour care delivery and collaborating closely with our interdisciplinary team (IDT). Your expertise will contribute to enhancing the physical and mental well-being of our participants, allowing them to thrive in their own homes.

Shift:
Monday - Friday, 8:00am - 4:30pm, 40 hours per week

Pay:
Starts at $50/hour

Basic Qualifications:

• Master’s degree in physical therapy from an accredited university (or bachelor’s degree and certification in physical therapy for applicants who graduated prior to 1/1/2003)


• Licensed as Physical Therapist in Indiana and Kentucky - preferred


• Minimum 2 years of experience working as a Physical Therapist


• Minimum 1 year of experience working with the frail or elderly


• Current driver’s license and proof of auto insurance
