Quality Improvement Coordinator



The Quality Improvement Coordinator supports the development and implementation of the annual quality improvement program.. This includes the identification of program-wide and center-specific quality indicators, data management, education and training of program staff around quality improvement, preparation of routine and special reports, and coordination of improvement activities arising from monitoring activities.


•    Participates in developing the Quality Improvement Plan (QIP) including annual reviews and updates.
•    Works closely with the Program Director who provides clinical direction of process improvement activities.
•    Prepares routine and special reports of quality improvement activities and performance.
•    Coordinates program-wide and center-specific quality indicators.
•    Assists in overseeing data management for QI activities including collection, entry, analysis and assessment of data.
•    Assist in planning and educating for staff on QI activities and techniques.
•    Assists in interface between consumers and program management for quality-related complaints.
•    Organizes and facilitates required committees, as per the quality improvement plan.
•    Participates in a variety of activities to ensure consistent program regulatory compliance and adherence.
•    Participate and attends relevant meetings and/or joins appropriate committees and workgroups.


Required Qualifications :
QUALIFICATIONS:

•    Bachelor’s degree required.
•    Minimum one to two (2) years’ experience working in the healthcare quality improvement preferred.
•    Must have flexible working schedule, occasional work on Saturdays.
•    Basic Life Support certification required.
•    One (1) year of experience working with the elderly required.
•    California Licensed Vocational Nurse or California Registered Nurse license preferred.
•    Clinical background preferred.
•    PACE experience preferred.
•    CPHQ certification preferred.
•    Knowledge of Medicare and Medi-Cal benefits and regulations is a plus.
•    Experience in project management, quality, and research preferred.

LANGUAGE:


•    Must be able to fluently speak, read and write English.
•    Fluency in other languages are an asset.
