Registered Nurse Home Care Coordinator – San Leandro, CA
Requirements to Apply:
- Certifications: Current CPR or First Aid training
- Licenses: Valid CA Registered Nurse License, CA Driver’s License for Home visits
- Education/Degrees: ADN or ASN required, BSN preferred
- Years of experience: 1 year of experience in a home health agency, primary care clinic, or health facility.
The Registered Nurse, home care coordinator is responsible for conducting periodic in-person assessments, conducting supervisory visits, providing skilled nursing care to participants, implementing primary care orders, evaluating participant complaints in person and/or by telephone, developing appropriate plans of care, providing participant, caregiver, and family teaching and communicates participant’s condition to primary caregivers, physician, and multidisciplinary team.
SCHEDULE: Monday - Friday 8am - 5 pm with on call and weekend rotation required
The requirement for the weekend shift is a rotating day on the weekend based on the availability provided by the clinician. The on-call requirement is also rotating and based on the availability of the clinician. Candidate will provide at least 5 days per month that they are available for on-call/weekend shifts and list other dates that they are not available. When candidate works a weekend on call they are entitled to an off day during the week. That date will be decided between the clinician and their supervisor.
Pay rate: $43.31 - $64.96 DOE.
Sign on Bonus: $5,000
Responsibilities:
- The Registered Nurse - Home Health participates in a multidisciplinary team; is responsible for care assessment, planning, and management of in-home care and home health care needs. This includes ongoing assessment of the home environment in relation to the changing needs of the participant.
- Manage the initial and periodic assessments of participants as assigned.
- Registered Nurse - Home Health is responsible for the presentation of any changes to the Interdisciplinary Team (IDT) for discussion and approval of any proposed change in the Plan of Care.
- Provides direct nursing care in the home. Develops and implements nursing care plan as needed and arranges for needed nursing equipment and supplies.
- Provide on-site supervision and instruction of Home Care Aides in the participant's home. Makes supervisory visits to each participant with a Home Care Aide every two months at home.
