LEVEL OF CARE (LOC) RN 


West Los Angeles, CA

FULL TIME DIRECT HIRE (not a temp or contract role)


SCHEDULE: Mon-Fri 8:30am-5pm 


SALARY: Up to $108k


WORK-LIFE BALANCE: NO Nights, Weekends, or Holidays and NO On-Call Shifts!


 


Must meet the following qualifications to be considered for employment:



  • FLUENT bilingual Spanish/English skills.

  • Graduate of a school of professional nursing (ASN, BSN, MSN).

  • Licensed as RN through California Board of Registered Nursing.

  • Verification of completion of CPR (BLS) and first aid training. 

  • Valid California driver’s license and reliable vehicle.

  • EXPERIENCE: At least one year as an RN, preferably in home or community health care working with the frail or elderly; experience in case management; and ACUTE CARE experience with IVs, bladder scans and assessing/triaging patients.

  • Comfortable and confident in providing in-home care for frail and elderly patients.

  • Proficient computer skills (MS OFFICE), and strong communication skills.

  • Candidates should be aware that the organization currently maintains a policy requiring employees to be fully vaccinated. 


Under the supervision of the RN Clinic Supervisor, the Level of Care RN conducts LOC visits at private homes with prospective enrollees and completes LOC documentation to send for approval by the State. Responsible for providing direct and indirect nursing care to assigned PACE (Program of All-Inclusive Care for the Elderly) participants.  Works in close coordination with the RN Clinic Supervisor and the Primary Care Physician to manage ongoing care and complex clinical situations. Supports the operation of the Clinic. 




RESPONSIBILITIES:




1. Attends Interdisciplinary Team meetings to communicate prospective enrollee cases for approval as needed.


2. Conducts initial home visits to carry out nursing evaluation for Level of Care assessment.


3. Completes initial and annual Level of Care assessments and sends to the State for approval; handles corrections and updates, as needed.


4. Records participant assessments, reassessments and care delivered in the medical record.


5. Reviews all necessary documentation and coordinates with other disciplines as needed for bi-annual LOC audit preparation.


6. Remains attentive to participants’ health status and social functioning; assists with identification of overt problems/needs and sets priorities; treats problems requiring immediate referral and/or follow-up; and interprets and records latest diagnostic results, as indicated.


7. In coordination with the Interdisciplinary Team and as assigned by the Clinic Supervisor RN, helps develop a Plan of Care based on nursing process that incorporates immediate and continuing care needs and includes the participant and family perspective.


8. Provides direct care as assigned by the Clinic Supervisor RN, including, but not limited to: wound care, IV therapy, blood sugar checks, IM injections, vital sign checks and/or follow-up on an episodic problem previously identified by the Primary Care Physician, such as Congestive Heart Failure, chest pain, dehydration, edema, urinary tract infection or other health problems.


9. Consults with and educates the participant, family members and/or caregiver regarding the disease process, self-care techniques, prevention strategies and medical interventions, as indicated.


10. Provides the Primary Care Physician with a summary of health findings and any changes in a participant’s health condition.


11. Ensures that all tests, lab work and diagnostic studies ordered by the Primary Care Physician have been carried out, as assigned by the RN Clinic Supervisor.


12. Coordinates all medication for BCSC, including the stocking of non-prescription medications, the ordering of prescription medications, the administering of medication in the PACE Center, and the monitoring of participant compliance.  Arranges for delivery of needed medications and supplies to the participant’s home.
