Requirements to Apply:


  • Current and Unencumbered RN license in the state of New Jersey

  • Graduate of an accredited school of nursing - ADN or BSN accepted

  • Minimum of 3 years as an RN within the last 3 years

    • Minimum of 1 year working with Frail or Elderly population preferred



  • Must have reliable transportation, a current driver’s license, proof of auto insurance and safe driving record.

    • Must be willing to visit Participants of the PACE Program in their homes if requested to do so.

Shift Time: 8:30AM to 4:30PM

Pay Rate: $41.00 to $52.44 per hour (Depending on experience and position type)

The PACE Registered Nurse (RN) has the responsibility for providing direct and indirect nursing care to assigned participants in a PACE Center, or in the participants home if needed. The PACE RN works under the direct and indirect supervision of the PACE Clinical Services Supervisor or a designee, but is independent in the application of advanced nursing knowledge and skills. The PACE RN manages complex clinical situations for which they are responsible.


 


Responsibilities:


 



  • Assessment: Initial and periodic assessments at least every 6 months. Collect participant data and complete required forms with appropriate responses according to department and industry standards using observation, critical thinking skills, etc. Identify participant's overt needs and concerns and sets priorities, reports problems requiring further referral or follow up.

  • Care Planning: Develop a plan of car based on nursing process incorporating plans of other disciplines and continuing care needs, including that participant and family in development and refinement of plan. Makes referrals to interdisciplinary support services. Communicate participant changes to team members.

  • Care Management and Implementation: Be present when medical services are provided; ensure quality of care in alignment with nursing practice standards and organizational standards. Provide correct treatment and procedures, etc. as prescribed. Provide education to participant family and team in the coordination of 24-hr care delivery. Identify emergency situations and independently initiate advanced therapy. Act as a primary coordinator of care with other health care personnel and evaluate patient care measures instituted. Work with PCP to furnish the preventative, diagnostic and therapeutic services as appropriate. Understand and demonstrate respect for patient rights and utilize established mechanisms for management of ethical issues in participant care.

  • Evaluation: Document treatment accurately and timely within the participants chart. Record participant care delivered as planned and with any variation including appropriate rationale. Make and record observations, as well as implement nursing measures related to impending or associated problems. 

  • Leadership: Delegate and supervise the provision of direct participant care by appropriate personnel. Integrate cost effective measures into nursing practice. Recognize concerns on unit and take responsibility for documentation. Demonstrate active participation in QI process; comply with organizational expectations to meet staffing demands based on participant care needs. Comply with safe working conditions and identify and take action to resolve any issues. Fulfil call for unit as assigned and adjust staffing volume for acuity as necessary. 

  • Interdisciplinary Team (IDT): Participate as an active and cooperative member of the IDT Team. Attend staff meetings and take part in participant care planning. 

  • Additional duties and projects as assigned.
