Overview: 


Responsible for assessing peoples supported health problems and needs, while developing and implementing nursing care plans, and maintaining medical records. Administers nursing care to people supported.


 


 


Essential Duties and Responsibilities:



  • Provides a minimum of 1.0 or 1.5 hours of nursing service per week per person supported as specified in the State and Federal regulations (Title 22).

  • Directly assesses each person supported individually once per week at minimum or as needed. Utilizes critical thinking skills while providing individual assessment and acts in a timely manner to coordinate continuing care.

  • Performs role of Charge Nurse at the beginning of the shift. Coordinates and or attends shift report ensuring LVN to LVN reporting.

  • Supervises LVN clinical responsibilities and determines LVN assignment to designated homes for shift and supervises nursing care.

  • Provides telephone or on-site consultations to Licensed Vocational Nurses as needs arise.

  • Assures quality of care by adhering to nursing standards of practice.

  • Delegates tasks to LVN's and Direct Support Professionals/Caregivers based on the legal scope of practice and on the preparation and capability needed in the tasks to be delegated.

  • Effectively supervises nursing care being given by subordinates.

  • Develops and assures implementation of a Nursing/Health Care Plan for each person supported based on a comprehensive nursing assessment and nursing diagnosis with reviews at least semi-annually or when changes are needed.

  • May be required to accompany people supported to medical appointments which may require RN intervention and advocacy.

  • Serves as a member of the Interdisciplinary Team at the Individual Service Conferences to assist in the development of comprehensive 30 day annual, and semi-annual ISP’s including identification of goals relating to the nursing diagnoses.

  • Works closely with the attending physician to secure specialist to meet the health care needs of the person supported.

  • Completes monthly progress notes based on Nursing/Health Care Plan, physician orders, and all persons supported change in condition.

  • Completes quarterly nursing examinations.

  • Oversees LVN's participation in monthly or bi-monthly doctor rounds with the physician and/or physician's assistant. Reviews all new physician orders with the LVN and ensures implementation of the new orders.

  • Oversees and verify LVN work with physicians’ orders and medication regimen every two-weeks (N) or every 30-days {H)

  • Responsible for making sure all persons supported have correct physician orders.

  • Interacts effectively with members of the nursing department, direct support staff, and other facility staff to keep them informed of persons supported medical needs, precautions to be taken, and observations to report.

  • Oversees the LVN in the review of medical records to assure current, up-to-date medical information is maintained. Works with medical Record Consultant to maintain records in an orderly fashion.

  • Participates as a member of the Admission/Discharge Committee and Human Rights Committee.

  • Oversees LVN work with the pharmacy consultant to establish procedures and medication reviews.

  • Completes pharmacy audits as required by State and Federal regulations. Maintains destruction log of expired and discontinued medications per established procedures.

  • Review’s documentation of medications dispensing, and drug regimen of each resident as required by regulations.

  • Assists in identifying and securing necessary equipment to carry out nursing/health care needs.

  • Works with dietary consultant and dietary services to review menus, emergency supplies, storage, and handling.

  • Promotes effective parent/family relationships by reporting concerns in a supportive and constructive approach as well as progress made by the person supported in a timely manner; answers questions and acts as a liaison in an encouraging manner to provide appropriate services.

  • Provides in-service training in the areas of client health needs, emergency procedures and infection control.

  • Understands facility's fire/disaster procedures and participates in drills. Understands role to follow in care of an emergency.

  • Completes a minimum of one course per year from the DONA website as approved by Director of Health Services.

  • Performs other functions as requested by facility Director of Program Services, Administrator, or Program Manager.

  • Reviews/Updates Therapy during work hours, no exceptions, unless otherwise advised.

  • Attends mandatory monthly staff meetings.

  • Performs other duties as assigned to meet department and/or business needs.


Required Qualifications :

Education and/or Experience: 



  • Graduate of a Board of Registered Nursing approved school with a current California Registered Nurse's License with at least one (1) year of clinical experience as an RN. 

  • Possess supervisory and management skills.

  • Previous experience in the care and treatment of individuals with developmental disabilities is preferred. 

  • Valid and current California Driver License is required.

  • Knowledge of principles and processes for providing client and personal services. This includes client needs assessment, meeting quality standards for services, and evaluation of client satisfaction.
