The Social Work Practice Manager is responsible for the administrative and business management of Social Services for the elderly including the planning, implementing and improving of social services with an emphasis on providing quality participant care. The SWPM supports the center operations in recruitment, hiring and onboarding of social workers. The SWPM also provides clinical leadership to train, and develop technical expertise of social workers along with Social Work Leads that add value and are aligned with organizational performance and quality measures. The Social Work Practice Manager is responsible for collaborating and integrating mental health networksclinical and other community resources into the PACE Social Services departments.

The SW Practice Manager oversees the Social Workers at the following locations: Oakland, Berkeley, San Leandro, and Concord -  and is expected to conduct center visits as needed to perform job duties


 


 


DUTIES AND RESPONSIBILITIES:



  • Leads process improvement efforts with Social Work Leads that support evidence-based practices and quality initiatives by troubleshooting, setting priorities, and implementing job assignments and expectations.

  • Develops, implements, audits and communicates clinical practice and policies and procedures for best practices, evidence based practice standards, and provides input to the Social Work Leads in the evaluation of social worker personnel at each location.

  • Utilizes quality improvement tools to collect, analyze, interpret, and display data that showed improved processes and patient care and reports during Quality Management meetings.

  • Maintains own clinical awareness and competencies in order to assure competency of staff and mentor staff to improve clinical performance and problem-solving skills.

  • Collaborates with the Social Work leads and center operations staff by responding and resolving participant care issues and grievances.

  • Collaborates with other disciplines and functional managers as needed and acts as resource to Interdisciplinary teams across the program.

  • Seeks out opportunities to build relationships with functional managers and other people resources who can contribute to the success of department's initiatives. Identifies opportunities to improve service delivery and/or program growth.

  • Contribute in the recruitment, screening and hiring of social worker staff to ensure clinical competence, as needed by Center leadership.

  • Assesses and maintains a Social work staffing plan that efficiently and effectively utilizes the appropriate resources ensuring competency, skill mix and adequate number of staff.

  • Manages clinical orientation/onboarding for social workers to assure orientation and onboarding for staff is timely, appropriate and comprehensive to meet the needs of the program.

  • Provides functional and technical social work consultation to Social Work Leads and Center Operations. Acts as subject matter expert in the development of performance improvement plans.

  • Develop, improve, oversees training and monitor clinical workflows for efficiency, quality and satisfaction including documentation in the electronic health record.

  • Coordinate, manage and evaluate student/intern placement with corresponding schools, universities as needed.


 SKILLS AND ABILITIES:



  • Excellent clinical skills, as well as expert knowledge of PACE care delivery model and Interdisciplinary team function.

  • Ability to engage and influence individuals and teams to improve performance.

  • Excellent verbal and written communications including demonstrated strong interpersonal and presentation skills.

  • Creates a team environment that encourages collaboration, staff participation and ongoing feedback.

  • Ability to work independently with minimal supervision.

  • Bilingual abilities a plus.

  • Maintain safe working environment by following Safety Policies & Procedures

  • Computer literacy, particularly Electronic Health Records (EHR) software(s)Microsoft Word, Microsoft Excel, and Microsoft PowerPoint.

  • Microsoft Office programs and project management software as needed.

  • Knowledge of relevant regulations from agencies such as CACFP, OSHA, California Department of Healthcare Services (DCHS), Center for Medicare and Medicaid Services (CMS) – PACE Regulations.


 


 


Required Qualifications :

MINIMUM QUALIFICATIONS:



  • Master’s degree in Social Work from an accredited university required.

  • Geriatric social work experience working with frail elderly of two (2) or more years required.

  • Experience in working with Medi-Cal, Medicare and PACE regulations.
