Social Worker

Location: Alpine, CA 91901
Schedule: M-F 8-5 -onsite
Pay: $31.52 - $42.47 per hour

Requirements: 

• Bachelor's degree in social science, Social Work, Psychology, Gerontology, Sociology, or related field

Position Summary:

The Temp Social Worker will provide case management and support services to patients, ensuring they receive the necessary resources and care coordination in a healthcare setting.

Responsibilities:

• Conduct assessments and develop individualized care plans.


• Provide counseling, support, and resource referrals.


• Collaborate with healthcare providers to ensure patient needs are met.


• Assist with discharge planning and transitional care coordination.


• Document case notes and maintain patient records.


• Advocate for patients and their families regarding healthcare and social services.


• Facilitate support groups and educational sessions as needed.


• Ensure compliance with policies, procedures, and healthcare regulations.


• Participate in interdisciplinary team meetings.


• Perform additional duties as assigned.

• Bachelor's degree in social science, Social Work, Psychology, Gerontology, Sociology, or related field 


• Strong communication and interpersonal skills.


• Experience with case management and patient advocacy.


• Ability to work independently in a fast-paced healthcare environment.


• Bilingual (English/Spanish) preferred but not required.
