JOB DESCRIPTION
POSITION: Social Worker, MSW
LOCATION: Fort Gratiot, MI
Salary - 71,838.00 to 83,945.00 (Depends on experience)

BRIEF SUMMARY OF PURPOSE OF POSITION:
Social Worker, MSW interviews clients and their families and coordinates and plans activities to meet their social and emotional needs. Provides crisis intervention to assist families in understanding the implications and complexities of the life situation and its impact on lifestyle
RESPONSIBILITES AND DUTIES:
- Responsible for assessing needs, analyzing the environment, coordinating and arranging services using person centered planning, assisting in the initial and ongoing Medicaid application/eligibility process (as applicable).
- Complete required forms, maintain files and related paperwork in a manner consistent with State, Federal, and Agency quality standards/policy/procedure/ practice/guidelines.
- Works with nursing facility residents, their allies, and facility staff to identify candidates for transition to community based living.
- Educates each participant regarding program eligibility requirements and refers persons ineligible for services to alternate community services.
- Providing mental health counseling to individuals, groups or families—if one is a clinical social worker
- Conducting initial assessments of clients' situation to determine needs and goals
- Researching and advocating for appropriate public assistance resources for clients
- Communicating with clients' care teams
- Providing crisis intervention as needed
- Referring individuals to appropriate treatment centers, as indicated
- Ensuring that all case files, and other records, strictly comply with policies, regulations, and procedures
- Coordinating treatment planning and maintaining ongoing contact with outpatient providers for the continual care of patients
- Actively participating in ongoing training as needed in order to meet all certification standards and credentialing policies
- Develops, implements and documents activity related to participant care plans following the assessment and/or reassessment, which includes but is not limited to:
- Providing information, education, and assistance to participants and families regarding their identified needs and services
- Make referrals to other agencies and/or fund sources as necessary, including Adult Protective Services for allegations of abuse/neglect/exploitation.
- Case consultation with team members and/or others to determine how to best meet the needs of participants.
- Coordinate and monitor informal supports and other agencies in meeting participants’ identified needs.
- Identify service gaps in individual cases and develop solutions to fill such gaps.
- Evaluate the effectiveness of identified goals and objectives.
- Strong participant advocacy, research and problem solving skills.
- Negotiates and arranges for the provision or health care and health-related services as prescribed in the care plan, including formal and arranged services.
- Works with family members and other informal caregivers, such as neighbors and community volunteers, to arrange for services and interventions as prescribed in written, person centered plans of care.
- Monitors the provision of services and assistance as prescribed in the care plan
- Performs participant monitoring and follow up as required or indicated.
QUALIFICATIONS AND SKILLS:0
- Licensed (State of Michigan) master’s level social worker (LMSW)
- Strong interpersonal, written, oral communication and computer skills; proven ability to communicates with diverse audiences
- A working knowledge of third party reimbursement mechanisms and an understanding of the social service delivery system.
- Must be able to read and interpret documents such as Medicaid policies and interpret supporting documentation.
- Working knowledge of public programs, including local community health and social service network required.
- Valid CPR/AED certification.
- Ability to become and maintain MMAP Certification (Medicare Medicaid Assistance Program).
- Ability to communicate with elderly and disabled participants on a one-to-one basis, analytic and organizational abilities, interviewing and listening skills, assertiveness skills, computer skills (for initial information set-up of services & on-going monitoring), and ability to work effectively as a member of an interdisciplinary team.
- A minimum of two years of experience in a human services agency, with preference given to those with prior experience working with the elderly and/or disabled.
- A working knowledge of third party reimbursement mechanisms and an understanding of the social service delivery system.
- Must be able to read and interpret documents such as Medicaid policies and interpret supporting documentation.
- Working knowledge of public programs, including local community health and social service network required.
- Valid CPR/AED certification.
- Ability to become and maintain MMAP Certification (Medicare Medicaid Assistance Program).
- Ability to communicate with elderly and disabled participants on a one-to-one basis, analytic and organizational abilities, interviewing and listening skills, assertiveness skills, computer skills (for initial information set-up of services & on-going monitoring), and ability to work effectively as a member of an interdisciplinary team.
- He/She must be willing and able to travel within all ten counties of the region (and at times out of the region area) in reliable transportation.
- Computer experience is necessary

OSHA CATEGORY: It is unlikely that a person in this position in the regular course of business would encounter tasks or situations involving exposure to bodily fluids.
