Location: Sacramento, CA


Schedule: Monday – Friday


Pay: $35.10- $43.27 (Depending on Experience)


Requirements: Master’s degree in social work (MSW) from an accredited program


Job Type: Temp 3-Month Assignment (Monday – Friday)


Position Summary:


The Social Worker is responsible for assessing, planning, and coordinating social work services for participants and caregivers. This role involves conducting psychosocial assessments, developing care plans, and acting as a liaison between participants, families, and the interdisciplinary team (IDT) to ensure quality care and support.


Responsibilities:



  1. Conduct initial and ongoing psychosocial assessments of participants.

  2. Develop and coordinate care plans with the interdisciplinary team.

  3. Provide case management and crisis intervention services.

  4. Facilitate participant care conferences, family meetings, and discharge planning.

  5. Manage hospitalizations and respite care coordination.

  6. Refer participants to community resources and assist with Medicaid eligibility.

  7. Maintain accurate and timely documentation in electronic medical records.

  8. Provide behavioral support and intervention strategies for participants with complex needs.

  9. Conduct annual home visits and additional visits as needed.

  10. Participate in team meetings and training to ensure high-quality service delivery.


 


 


 


Required Qualifications :


  • Master’s in social work (MSW) required

  • Minimum 1 year of healthcare experience, preferably with elderly or frail populations

  • Strong case management and care coordination skills

  • Ability to work in a fast-paced, interdisciplinary team environment

  • Excellent communication and problem-solving skills

  • Bilingual (English/Spanish) preferred

  • Proficiency in electronic medical records and documentation
